I understand why karev didnt kiss izzy. When he was a teenager that blonde girl talked shit about his mom and it hurt him, he was afraid of getting close to her and afraid of getting hurt. But you could tell he really wanted to kiss her but he was worried he'd mess it up or she would end up hurting him.
