I love how he's just completely flabbergasted when she tells him she's pregnant. She didn’t need to do a fellowship because she did a general surgery residency, so as soon as she finished fifth year she became an attending.

Fellowships aren’t exactly required. They’re recommended but not required. So once Baileys residency was done she just became a general surgeon

Usually you tell which gender by 20 weeks so she might’ve been 4 months or over

She said she took the test a month before her fellowship starts so I’d think 2-3 months. As a dr she has access to get the early gender test to know what she’s having.

the hospital is a large metropolitan hospital that has some of the best doctors in the country of course they have always had fellowships

No.. They didn't. Meredith and the other interns instantly became an attendings. It was Meredith ideal to make a fellowship, so Jo could help with the mini livers. It could have been a plot error.
