It's probably the definitive online resource on the topic.* However, he leaves out a crucial detail on how to calculate the width of the hinge, so I've included the formula below.
width of hinge = (3x the thickness of the cover boards) + (1/4 the width of the spine)
*Caveat that I know Michael and may be biased.
