Well this may not be correct but an alternate reality can be formed from a alternate reality so what I think happend is this Doc Oc is from another reality in which Doc Oc never came to his senses. Misterio not died and if you carefully notice the teaser traller that Inside the home every where is Ice and if you notice that peter is not suffering from cold and no smoke is near their mouths..And behaviour of Dr strange is also different Which clearly shows that it is the illusion of Misterio...
