I'm pretty sure #2 is the Slit Mouth woman from Japan. The story there goes she has a slit across her entire face from ear to ear, not in the shape of a permanent smile, just a slit that still has blood pouring out of it. She does the whole bit with the surgical mask but the only way to get away alive is when she shows you her face without the mask, you have to confuse her by saying "you look normal" or something along those lines, and while she's confused, you run away as fast as you can. It is not specified that she only goes for children. There is a story that she was the most beautiful woman in the village and the wife of a samurai who thought she was unfaithful because all the men in the village liked her and wanted her to be theirs because of her beauty. So one day the samurai took his sword and gave her a gruesome slit from ear to ear, and said "no one will think you are pretty now" before dumping her corpse in a nearby well. I think it's because they latch onto things that had sentimental value to them when they were alive. Since it's often children who posses these dolls so it's completely reasonable for them to feel very strongly towards the dolls. And for some grown ups dolls are important reminders of childhood. I don't know if this is correct or not but its just my theory :)

Actually the legend of Red Cape goes that if you asked for Blue he would beat you or suffocate you until you were that color (more likely you would still die) and red well, he'd slash at you until your clothes were red in your own blood (Again you would likely die).
