It's something that is linked with the mental side of the game. He lacks composure, awareness, and intelligence in his game. I would seriously suggest liverpool try a simulator with him.

I think he is running alone outside as his fitness regime would be focused on light training while recovery.....nothing too hard to niy exarcebate his injuries......PLUS he might take the time to clear his head.......I remember VVD during his own recovery...he was always challenged mentally while going through his recovery...seeing the rest of the squad training.....
