when i use cascade policy so it delete appointment with patient even the appointment in done state where we have already override unlink method for that why ?

Thats True But if you tried to delete appointment from appointment view that will raise error" done state can not be deleted" Here your unlink method work From Patient model if you delete the patient the ondelete="cascade" will take effect and delete the appointment You shouldn't use cascade in scenarios where you have to avoid deleting records

I believe that 'unlink' method must be triggered by view/user. However 'cascade' will bypass 'unlink' method and deal directly with the database, that's is why appointment in done state will be deleted too.
