Have you tried using the thprac centipede card fix? I don't know if it works for extra stage replays, but it makes the main game replays with centipede card work correctly.

You can use the thprac centipede bug fix to watch your replay just fine You need to have thprac applied on your game and once you do that press f12 to enter the bug fix menu to solve your centipede problems
