Whoever writes your stories needs to learn a little bit about basic medical emergencies. Internal injuries with bleeding ALWAYS require emergency surgery if the bleeding is to be stopped. Also, renal failure does not affect medicinal regulation - that's what the liver does. Renal failure can be dealt with by using a dialysis machine. People live with that for years. Yes, they can and do die, but not so quickly. Also, chronic malnourishment would weaken everything and complicate any and all emergency situations, making her weaker and at high risk for complications. With severe internal injuries, the doctors would be looking for blood, not a kidney. Dialysis can tide a patient over until they've got bleeding under control and stabilized the patient. No reputable hospital, nor any doctor worth his salt, would ever even consider accepting an organ donation from anyone with a heart condition. You won't get rejection signs during the surgery - it's not possible. It needs time to actually get attached and the body to actually register it's there first. Bad news that might cause shock is always withheld until the patient is safely stable. When a heart stops, they all go into doing CPR immediately.
