Some air craft enthusiasts (not me) have explained what "visual separation" meant, and it typically means using just pure human vision to separate your own air craft from another one in the sky. The heli pilot asked permission to use "visual separation" when it is night time and in an area of congested civilian planes. Using just your own eyes to separate between planes instead of the "highly praised" technological instruments on board can only suggest the pilots WERE training their ability to navigate with their own eyes at night. But how do you accurately navigate when you don't know which is the right plane to look for, nor see how far you are from the ground. Thus the pilot saw the wrong plane and is flying way too high.

They were flying above their assigned ceiling. We may never know why. They also reported having visual on the CRJ, when -apparently- they didn't actually have eyes on it. We can only speculate that they were looking at the wrong plane. That was all it took to cause the collision. Everything else was done by the book, which tells us that the book needs to be revised. (The ATC didn't seem to know that that the helo was out of its assigned flight path; the 200 ft ceiling should be programmed into the radar system so that an excursion triggers a warning.)

More likely they were looking straight ahead at the next plane in line to land, not at a plane taking off and headed away from them. The CRJ was above them and to their left - either they didn't see it at all (wearing night vision goggles, maybe?) or didn't appreciate that it was going to turn in front of them. (You have a pilot and a co-pilot; for safety in a crowded sky, I think one of them should _not_ be wearing the goggles so as to maintain a full field of view.)

When you’re wearing nvg at night all lights out there are super bright and you have no peripheral vision. At 8:45 when the TOWER alerts the Black Hack Pilot the Jet is approaching, at 8:47 the Tower asks the PAT25 to confirm, do you have the CRJ, in sight? And seconds later the Tower told the PAT25, to pass behind the CRJ that moment impact had happened. It seems that many things failed. First, there were 2 or 3 minutes between the Tower alert and the impact happened when ATC asked the Pilot to keep insight into the approaching JET someone was not doing the math correctly intentionally or unintentionally, the jet was coming, the Helicopter was in the wrong altitude and the middle of the river, ATC told him to go behind it if he sees it but in this case, the Helicopter not stopping there to give way to the AA5342 the Black Hack was flying too if he has to go around it, that time he doesn't know where was the JET and if he sees it can he go deep East? he has restricted path and This is a short time and maybe he was in a level and got abstracted by the city light and couldn't see the right flight.

Good man Mr Rose, you've spoken the truth and highlighted the main root cause of this tradgic accident. No matter the other contributing causes (Helo pilot too high), this is the basic problem. A nominal vertical clearance of 200 feet is, as you said, unacceptable. The airspace at the end of this active runway needs to be redesigned, (no traffic passing 200ft underneath landing aircraft), to make it sterile for aircraft with landing clearance, appoaching the runway, so that this never happens again!!

? Have you ever flown with nvg’s in a city around an airport? That alone is difficult let alone doing it thinking the aircraft your looking at is the one you need to to worry about while actually flying in the direction of the one that you should be worried about. Nvg’s have no peripheral vision. They probably didn’t see it until it was directly next to them and that’s too late due to the altitude they were at.

What happened to the Crash Avoidance system?? It usually Yells at the pilots" Crash Imminent turn left or right now depending on positions of detected aircraft, the system works out the appropriate action & the pilot must respond instantly to avoid a crash. This is my understanding of the system which works this out based on other Transponders running in the area, unless someone didn't turn their Transponder on which is Illegal at least in Australia, which would result in one CA system not being aware of the other aircraft and think things are all good. I'll add on seeing the radar video the Transponders had to be both on for the radar to know who is who. I always want to know what went wrong in these crashes

You're talking about TCAS, which doesn't alert when an aircraft is under 1000 feet, which was the case here. The reasoning has to do with the low altitude, and the assumption that a plane that's at low altitude is landing or taking off, during which time there's expected to be lots of relatively close aircraft traffic and therefore the constant alarms could be distracting to a pilot while trying to land or take off. Usually at such low altitudes, air traffic control will assume a greater responsibility to ensure that any aircraft remain separated.

I guess it really comes to the fact the helicopter was flying across a circuit path & at the wrong height apparently. In Australia I've flown both helicopters & planes and neither are allowed to cross a circuit path at an airport. It's join a circuit with appropriate radio calls or not be there no matter what aircraft your flying. Well I await the report because they won't miss anything hay. Thanks for putting me straight on that, I actually had forgotten about that for the reason above.

Interesting fact: I see a lot of people are sad and mourn the loss of these people but just the fact is: 67 People died in this plane crash ✈️💥 45,936 People died in Palestine 🇵🇸 The Palestinian people ❤❤❤ had families, dreams & goals, and now their homes and their losses and their lives.
