I suspect it had 2 slugs to clamp the quill from both sides.

Brian that's an Aha moment, I believe you're correct. On my Monarch the quill clamp hole is blind and there's only one slug, so I was stuck in that paradigm.

Could there have been a threaded scalloped plug then the upper scalloped piece had a through hole for the thread to pass then when tighten the knob then pinch the quill ? Thanks for the video ed

Yeah, you and Brian Block nailed it. My Monarch has a blind quill lock hole and one scalloped plug, so my mind was stuck in that mode. Two threaded quill lock plugs on the to-do list now.
