Sure, the modifier definitions found in the CPT appendix A : Modifier 59 should be used when coding for a different session, different procedure or surgery, different site or organ system, separate incision/excision, separate lesion (noncontiguous lesions in different anatomic regions of the same organ), or separate injury.

Time and MDM? For the exam, if time is documented use time. In the real world you would have to follow your employer's requirements.

My question is concerning the 13-year-old with the left tibia fracture. The documentation states that the radiographs were ordered, performed and billed in the office already.
