I have been worried about the lack of respiration but those deep cardiac compressions make sense in terms of generating a negative pressure so the air sucks in...the things I worry about! Thanks for answering my dilemma.

I don't know for sure if the chest recoil brings in an appreciable amount of air (but it's better than nothing). I suspect that the lack of delays in compressions AND preventing the avoidance of providing any intervention in people who think mouth-to-mouth is gross (I think it's gross) is probably the real reason it helps. My guess.

Dr Patwari when they give unstable patient next step is cardioversion or defibrillation before cpr in exam next step rigth

Asha, I'm not sure what you mean. In unstable patients with a shockable rhythm, we would give shocks when a defibrillator becomes available. Chest compressions continue until we're able to get that set up.
