If I am not mistaken you are duty bound to report to the Porte state authority investigative branch and the flag state too. In fact there is MGN564 gives instructions in the case of a British registered vessel world wide or a vessel in a British territorial waters who would have had an incident. Such agencies are present in many countries so I think that asking your agent in port would be the best way of obtaining the list of local entities to be informed. If there is a serious injury I am pretty sure that the police will need to be informed as they would want to conduct their own investigation (most probably also detaining the vessel until such is concluded) Other then that I also think you have to inform your DPA. Why are lifting appliances and lashing (chain register listed equipment) inspected by a competent person and not class? Is it because it an ILO requirement?
