I do watch for hours....every shift if and when it slows down I usually listen to this lecture and Tachydysrhythmias & Bradydysrhythmias

My 19 year old grandson had debilitating chest pains on and off for 6-12 months. His father is only 44 and has advanced atherosclerosis, so I took him to a cardiologist. My hat is off to that doctor because he took my grandson’s chest pain very seriously. He immediately scheduled a treadmill stress test for my grandson as well as blood work. Fortunately my grandson’s heart is strong and he was diagnosed with severe GERD. The cardiologist prescribed a protein pump inhibitor medication and my grandson’s chest pain went away. He takes the medication every day and if he forgets the pain returns. I was very impressed with the cardiologist’s taking his pain seriously and not discounting it because of his young age.

Thank you very much for this very informative lectures. You are not only making it easily understandable but most of all HILARIOUSLY presented. ....from a solo M.D/only doc--- in an isolated small island in a nowhere in the tropic.

Dr. Mattu, I love the inciteful and interesting topics. Very eye opening. I have a question for you or your staff concerning Adenosine. I understand why Adenosine does not work for A-fib, and why it is dangerous for A-fib RVR with WPW. The local EMS medical directors listed in the protocol book that WPW is an absolute contraindication for Adenosine. 1. An Internet search shows conflicting opinions on Adenosine use for excited WPW. 2. When questioning one of the medical directors, he explains that it is his "practice" in the ED not to give it for anyone with WPW, choosing other medications (not carried on local ambulances) to treat the SVT. 3. Yet the drug insert for Adenosine states it is indicated for "Supraventricular tachycardias, including WPW." (Assuming FDA has not withdrawn their approval.) I am trying to find empirical evidence if Adenosine is still indicated and safe for SVT with WPW. Any suggestions?

I think it would be very difficult to appreciate WPW in a pt presenting e SVT. And only after adenosine is administered, it will unmask the WPW.

Not a ED Dr…. Just a paramedic student. But my understanding is that if they’re symptomatic enough, skip the drugs and go for the synchronized cardioversion. “Unstable gets the cable” I know that doesn’t answer your question exactly, but as far as treatments go, you’re safer going with electricity with this particular patient. I think.
