Hello! "20 by 20" means that he is decreasing the target Min. Volume in stages of 20. In example in the video, Dr. Arnal sets the first decrease from 130 to 110

I have had severe sleep apnea for some time now and I got diagnosed with complex sleep apnea in Dec 2015. I've been using my asv bipap since Jan and have noticed little if any difference and my sleep scores are always over 90. How long should this take to get me to some what of a normal state?

The ASV you are referring to is Adaptive Servo-Ventilation (used in non-invasive home ventilation). This is not the same as the ASV (adaptive support ventilation) described in this video. As you can see the patient is invasively (intubated) ventilated in an ICU; not the same use case as yours :)

Yes, this video does not apply to you. Look up adaptive servo ventilation which is what you are probably on for your complex sleep apnea. Welcome to the lovely and dangerous world of medical abbreviations.
