Plz always mention the place from where you are talking to so that I can assume the weather at your place. But anyway, I think either your plant is getting water continuously more than that much is actually required or starving for water. In both the cases softness of caudex arise. In case of overwatering, you should repot the plant in a dry soil and avoid frequent watering. In later case feed water in little amount on regular basis.

What is stand stone ke tukde? Is it large granular sand or something. If I take 4 part normal garden soil instead of sandy soil and add river sand 1 part, cinder 1 part, vermicompost/leaf compost 1 part, and 1/2 part each of Neem cake powder and Bone meal , will it be perfect? Please advise?

Main purpose is to have a very well drained soil. So, you can ammend the recommended mix with available options.

It depends on the size of codex of the plant as I have already mentioned in the video. The size may be as low as 3 inches initially and gradually may increase up to 18 inches or even more.
