Dr. Russell Barkley. Adhd is closer to diabetes then it is anything else. If you find yourself becoming addicted you probably don't have adhd
@thePSYCHcollective2 жыл бұрын
The only similarity between adhd and diabetes is that the are both conditions which respond to treatments. In every other manner that are as different two conditions could be. Diabetes is the inability to dispose of glucose which causes its blood concentration to rise and thereby chronically will cause micro vascular injury (or acutely DKA). Not addressing diabetes with lifestyle (or meds) will damage the health of the diabetic. ADHD is a deficit in frontal lobe inhibitory capacity. Not treating it will mean one is distractable and can’t concentrate on uninteresting things. That has social, occupational and esteem consequences. As to addiction. If one has no other sources of positive emotion and chronically lots of negative emotion, one can become addicted even if one has adhd. Not everyone that is prescribed ADHD meds uses them as prescribed. And there’s plenty of overpresciption, tolerance and dose creep
@M.Shepardbee2 жыл бұрын
@@thePSYCHcollective @the PSYCH collective The core of ADHD is an emotional regulation deficit. The symptoms of ADHD are external regulation. Not addressing ADHD with medication and lifestyle reduces life Expectancy by 13 years if it persists into adulthood. Without medication the symptoms are self regulation. With medication less self regulation is needed. When medication is taken self regulation is less needed. Hyper active behaviors lessen, distractabilty lessens; the body does not need to work overtime to regulate the brain. The cognitive issues come into play because the medication is a powerful regulation tool. The ability to regulate internally was never there and will never be there. When you take to much medication you have a regulation issue in the opposite direction. You are not becoming resistant to medication, your are becoming accustomed not seeking external regulation. The slow increase is so you can replace one with the other. If you are tired and stressed, you will habitually try to self regulate. Your "symptoms" get worse and you feel better. You over extend yourself habitually, as you always have as a regulation tool. The cognitive issue is the lifestyle change, now that you have some free space you apply it to schedules... or not, because you never have. So some sugges 2 days off meds. Which feels good. Then back to the grind. Except you never allow your body to handle other things as it cannot trust that tomorrow the stimulus will be there. Are you addicted to insulin ? No you can regulate without. But what about the worst diabetes that you can't regulate. Well you die. Are you addicted to adhd medication? Well no you can regulate without. What about the worst adhd That you can't regulate. Well you die. The "addiction" implies that it replaces and stops the body from doing a natural thing with diminishing returns. What it does is allows the mind to be less distracted in a overstimulated society. Not being on meds is bad enough, putting someone into a on/off state is guranteed to cause accidents. Your life becomes habitually different, when on meds i trust i didn't forget to do certain things that I wouldn't trust without.

I’ve been diagnosed with ADHD since I was a kid. I’m an adult, in my 40s, and still haven’t been treated for it. I’ve been treated for depression, anxiety, bipolar 2, and nothing has worked for me. I can honestly say that after I tried adderall a couple of times, I felt that it was the only med that ever worked on me. I had no euphoric feeling whatsoever. The only thing that happened was I straightened out my messy desk. (Lol) I may be able to go on it sometime next year, but I have to get off this one other medication that doesn’t mix with adderall or other stimulants. The really depressing thing for me is, I’ve wasted away 40+ years because of these doctors being too afraid to put me on a stimulant med because of my past problems with addiction; I self medicated on alcohol and other drugs BECAUSE of my ADHD symptoms. Anyway, thank you for this video. It really helped me understand more about these meds.

This sounds very much like my experience. Jumped around from Bipolar to ADHD diagnosis and really only accepted the conclusion that I simply didn't like school or as others put it, "too smart for school" A cute way of saying 'easily distracted' No mood swings, I'm pretty well balanced morally and behaviorally, but I find myself in a writers-block state of mind sometimes. As a mechanic with literally ENDLESS amounts of work, I find that small doses of Adderall provide that minimal change in brain chemistry I need to go throughout my day without slowing down. Not necessarily moving too fast, but I find I have better mental clarity. Even when subjecting myself to doses of THC which should modify the effects somewhat of these kinds of stimulants, I still feel "right" after a day of Dextroamphetamine use. Which is interesting considering I am 32 years of age and have been unmedicated since my early teens after a nasty experience with Ritalin and overzealous prescriptions for my minimal issues.

Think very carefully. Look at the list of "secrets." Stimulants are very addictive. They can have a very positive effect in the short term, which gains these doctors all the glowing reviews that they use to sell people on their safety and efficacy, but nothing is for free and you pay for it on the back end with depression, anxiety, psychosis, and suicide. Going on and off these drugs is traumatic at best. A lot of people end up developing symptoms that lead to more severe diagnosis of mental health disorders that they never had in the first place. I'm just saying don't just take this doctor - who will profit from you believing him - at his word. Find out as much as you can and talk to as many people as you can. Once you go down the medication path, it's very difficult to get off of it.

@@thePSYCHcollective I wonder what percentage of people develop tolerance, and how quickly it develops. Very interesting. There may be other reasons people ask for a dose increase - increased fatigue due to sleep issues, or they realise the dose they were on initially wasn't high enough to begin with, or theres depression developing and they actually need that managed. In terms of recommendations of taking 2-3 days off completely per week, and 1 week off every 3 months, are there any studies to show this reduces tolerance effects? Id be recommending patients experiment, perhaps reducing the dose at times, rather than stopping all together. This would enable them to function looking after kids/socialise and do household chores/life administrative tasks. Anecdotal medicine is a tricky area to make recommendations from.

Yes. I don’t know the proportion, but it’s not trivial. Those who become tolerant end up with the predicament of being on maximal dose without efficacy. Forgot to mention each person is sensitive to different things so a different adhd medication could work better than another in regards to tolerance, another thing in regards to creativity I would not say it reduces it the medication seems to go through stages. Here is what I named the stages based off how I felt Stage 1 Massive Energy ( Had 50 Coffee's Feeling ) Stage 2 Alert ( Feeling Awake ) The feeling to get when you have been up for hours and your body is finally awake and ready Stage 3 Super Focus ( Not talktive ) You have to be doing something and you focus on it till complete Stage 4 Melo ( Feeling Normal ) You chill out and think things out better often get the feeling Im finally normal Stage 5 Come Down ( Fustrated or Tired ) You come down like 50 redbulls have ran out and you can get grumpy or super tired. As it kicks in I find that if you want to do something make sure you wait around 15 minutes or when you feel it is kicking in you then start doing the thing you want to do because that hyper focus seems to zone in on what ever your doing when it kicks in, I noticed that if im chatting away I actually end up wasting hours talking because I was doing that as it kicked in. Lastly if you dont care about focus because maybe you manage it fine and want the energy hit, this is the ( motivation issues ) I take less than my normal amount say 3 tablets is normal Ill drop it down to 2 as an example and this gives me that Im fully awake effect. I find normal doctors are crap with this you really need to speak to someone that understands these medications I have worked with a number of Teens that were told nothing about different adhd meds and what treats what better. I see the medication does not work a lot and they end up not trying, I am quite tired of watching normal people and parents not bother looking at proper medication and how to manage people with focus issues its not hard at all.

One thing that no one ever seems to talk about is the effect of caffeine combined with these drugs. My first six months on Concerta were pretty good, but the dose started creeping up, and at some point I just started upping my caffeine intake. This wasn't a conscious decision, it just kinda happened. Caffeine has always been a dismal stimulant for me, but this time, combined with daily Concerta, it really started to wreck me. It basically ruined my sleep, which in turn ruined everything else. I quit cold turkey and am now 10 days off caffeine. My sleep has improved, but I feel really depressed, unmotivated, tired and sluggish all the time. I have still been taking my daily concerta, but it doesn't seem to help much. I can't get anything productive done at the moment, so I was thinking about taking a longer break from Concerta as well. And no, I am not going to consult my doctor, because he doesn't know jack shit. If I wanted to up my dose, I should probably consult him, but I don't feel like it. I am torn, because the first 6 months on this medicine objectively improved my quality of life a lot. But now I feel like I wanna curse all stimulants to hell.

Some prescribers might suggest weaning then taking a break from them to rest the tolerance. Subsequently they could be used 4-5 days a week to reduce the accumulation of tolerance. (Not medical advise)

I wish I would have known all these things 3 years ago. Those are exactly the side effects I felt in all of my cells. At the end I had to quit cold-turkey. I slept almost 14 hours a day to recover for a month. I was awake at the night and sleeping during the day coz the day light was annoying me like I was a vampire. After 2 years of quitting i tried to take one Ritalin for my surgery exam. Yes it boosted me but then immediately gave me paranoia and then I never touched it and I hope I will never touch it again.

When I started I figured the weekend break would be a good thing to do, my doc said that it wouldn't be necessary with vyvanse given how it's released in the system. But the ups and downs were terrible. I feel much better and perform better with just one day. My main problem is how fast my body metabolizes it, so at best i get a solid six hours needed a boost of IR in the afternoon
@thePSYCHcollective9 ай бұрын
I might have misunderstood but it sounds like your doc suggested more of the drug to deal with the tolerance to the drug (whereas a break from the drug would reduce tolerance). If tolerance to the high dose is causing withdrawal come-down effects in the afternoon, the “solution” is even more of the drug. There’s possibly some dose creep going on which can add up to a lot of milligrams!

Why does the crash get worse overtime? Is this due to no breaks? If breaks are taken can you reduce the worsening crash and possibly have a stable crash consistently?
@thePSYCHcollective2 жыл бұрын
When we use a stimulant there is a rebound when it wears off. The higher the dose the more evident that it. For some people regular use leads to tolerance and increased dose. Worse rebound. A break can start to resolve the tolerance. But this is not medical advise, discuss this with your doctor.

I cannot find the answer to this anywhere, I'm on Ritalin IR, 20mg morning, 10 midday and 5mg before bed... But I'm so incredibly tired ALL the time, like I'm not crashing, i can take my Ritalin and have a nap 30 min later, hence the 5mg at night. The higher the dose the less need for sleep, but still this fatigue persists. I've tried researching this, I've spoken to my dr's about it and just cannot find the answer... Can you possibly explain this to me? Do i need to change meds? I do take medication breaks every 2 weeks, but it doesn't seem to make any difference. I'm less anxious which is great, but I'd love to not feel like i need to sleep 20 hrs of a day. Great video, so incredibly informative!
@thePSYCHcollective2 жыл бұрын
I can’t answer this as it would need to be worked through systematically and addressed empirically. Here are some comments: Odd to be taking it before bed, it’s mostly dosed morning and midday only. I’m curious as to the rationale for dosing before bed. Chronic fatigue/tiredness may relate to other matters such as CFS, Long COVID, etc. A long break might be informative. One needs to weigh up benefits/costs of any treatment.

Is der a relation between anti anxiety meds and stimulants? Does stimulants work less efficiently when combined with an anti anxiety meds?
@thePSYCHcollective3 жыл бұрын
Your question as posed is not really answerable because “Anti anxiety meds” is a massive category including almost all the antidepressants, all other medicines with sedating qualities and drugs which affect adrenal/epinephrine signalling (beta blockers, clonidine) and others. So it depends what exactly is being combined. Some combos are okay, some are not so great, some are not a good idea.
@Macleod15452 жыл бұрын
Do extended release stimulants have the same issues with tolerance as instant release?
@thePSYCHcollective2 жыл бұрын
They can. I’ve stopped initiating long acting as the MANY of my patients would find that tolerance and dose creep would have them maximum dose after 9-12 months (even with skip days). But SOME people don’t seem to get tolerant to neither long nor short acting. [just observations, not medical advise]

Tolerance to amphetamines was established in WW2 by the nazis who used it on their soldiers. They noticed that the dose had to go up for a large proportion of users. Tolerance is not a controversial notion for stimulant drugs such as amphetamines.

@thePSYCHcollective Жыл бұрын
@Evan Mc The difficulty is that one usually needs some evidence of ADHD prior to PTSD. The general wisdom is that ADHD is a developmental issue and persist from childhood. So doctors will usually need some supportive collateral information to justify the diagnosis (school reports, interview with parents etc). Not all psychiatrists prescribe stimulants. There are clinics that specialise in ADHD. They tend to be expensive and doesn’t guarantee it will be prescribed.

I started taking ritalin when I was 16 years old, 10, 15 and than 20mg 3times a day, plus a 10mg at the end of the day. This makes total 70mg dose of methylfenidate. The crashes when it worns of were terrible and that's why I tried xr. Now 17 on xr and hated it, I could sense the little effects comming up and comming down. This was for 2 weeks and just terrible. Still 17 year old and now on dexamfetamine 10mg 3times a day. Total 30mg of dex. The crashes are way less potent and almost even unnoticeable. Took it for 4 months and than 3 weeks I did not consume any stimulants. It helped a little but not much, btw not that I notice a difference in performance now in comparison to the first few weeks of taking dex. It was just to not get a tolerance faster. So after how many months from the start of taking dex will this "tolerance" be noticed?
@BenDover-ek1zp2 жыл бұрын
I'm not sure if he or anyone can answer that unfortunately. He said some people can find a stable dose and be fine without taking any breaks for years. But others can't and need to take breaks. PS: i'm also on dex and wish i knew the answer to your question.

Thanks for raising this important matter. Tolerance to dex necessitates gradual withdrawal and a period of cessation to resensitise one to the effect of the medication. Then after resuming, people who are prone to tolerance may benefit from time away from the medication (eg weekends off it and a week or so every few months) to limit accumulation of tolerance. Taking meth can have serious consequences and tolerance to it is likely if one has tolerance to dex. So one ends up back in the same predicament but with more difficulty stopping meth and more troubling health effects.

I have ADHD, autism, and a frontal lobe injury. I have so far tried over like 20 medications from all types and backgrounds. I have tried dextroamphetamine and lisdexamphetamine and they work for about 4 days and then boom dont work. its the same with all meds... dextroamphetamine required 30mg twice a day to work plus taking it with antacids and bicarb of soda. Theres a secret he didnt mention. Vyvanse took 2 x 60mg capsules in the morning and lasted for about 14 hours so not bad... Kinda shit... the best ADHD medication ive tried so far is Lyrica. Yet again. A dark rabbit hole of a drug.... Modafinil is probably the best ADHD medication ive tried, 400mg in the morning it takes and its way too expensive, heck knows why its not on label for ADHD coz it should be. Also I cant see how dexies etc are S8 drugs. Phentermine 45mg SR pisses all over dexies... I need to take 90mg in the morning another 45mg at 4ish and it will last till about 9ish.... BTW the only 'drug abuse' i ever did was smoke weed for a few years, took like 5 eccy pills my whole life... so nothing.... there has to be a better alternative. I am literally looking into the science of crystals and gemstones now... absoloutely baffles the 10+ doctors ive seen. No one can help so i have to do it myself... oh also heres a cool secret, umm if your ever feeling depressed, try 70%+ dark chocolate. that wowed me big time.. couldnt believe it. umm hair thinning? dont pay IHLS $3243254324 for a 250ml bottle of scalp stimulator, just dilute pepperming oil in a carrier oil .... hmmm i wonder if i can do a doctor consult with someone in the US or something? I know that the US are working on a nicotine based ADHD medication which is planned to replace the current 1st line ADHD meds. Oh theres another secret... Im baffled when i meet someone who has ADHD but doesnt smoke...
