This is to test a patient's responsiveness. Something you may have to use pain to get a response because other means aren't getting any results. It wasn't a threat she have to make the patient feel little pain

They're intubated - who cares. You can slap them around a bit for checking RASS. It's not like they'll remember it. Any real ICU nurse would know this.
