a fail criteria on the NREMT checkoff sheet is failure to initiate Oxygenation of the patient within 30 seconds of identifying respiratory arrest if we follow the sheet we go well above 30 seconds also when inserting a OPA in a Pediatric patient i was taught not to rotate do to the patients soft palate

1ST: this is respirator my friend no IO has been placed lol 2ND:Go back and review your skills because its clearly stated you are NOT supposed to rotate an OPA while inserting in a Pediatric and if I had a student do so I would "probably" not fail him (but could for incorrectly use of a basic airway) but I would ensure they knew the proper procedure before they go out and potentially harm an actual child.

@@eleogramable From my AEMT textbook, "In children, the only acceptable method of inserting an oral airway is to use a tongue blade to hold the tongue down while inserting the airway straight in. Because the airways of children are undeveloped, rotating an oropharyngeal airway in the posterior pharynx may cause damage to the uvula and soft palate." This means no 180° or 90° rotation with OPAs in pediatric patients. You must use the direct method, as you do with adult supraglottic airways. Jamshidis are inserted IO with a twisting motion, but that's not what we're talking about.

what do you mean? the NRB its due to the fact its training and no real o2 is being used but he should have verbalized it

In this case, since the child isn't responsive in the scenario, are we checking ABC rather than CAB just because that's what the skill sheet states?

Precisely, stick to those sheets exactly the way they're worded. The NREMT is usually slow to adopt new changes in general, mostly for good reason.

This pediatric patient has a pulse. General impression was hard time breathing and so it is ABC.
