from my interpretation of what she later came out and spoke about, i believe the hospital scene was in reference to her being sa’d, abused and left pregnant at 19 by a music producer at the time. she was threatened with having her music files destroyed and i think that’s why the manager called and told her to give up on her career. she suffered a psychotic break after the abuse but went on to try again. her coming back from that, rebuilding herself and trying to do music again leading to her meeting with her current label interscope. this video is the story of everything that lead up to her career. the song and video are so special and deserved so much more

The context for the videos concept is that in real life, Gaga had a traumatizing experience as a young woman, where she was held hostage & physically assaulted for weeks by a music producer, and when she fell pregnant, he literally dropped her in a weakened state on the street. As a result, and wanting to continue her music route, the goal of which gave her strength & focus, she of course, not wanting to carry his child compounding with a physical disorder that affects her nerves, had it, well, you know, while still convalescing in a trauma ward. The story is effectively her rebuilding herself from scratch. Thus the no intimacy for two weeks commented by her caregiver in the beginning. Later she would develop a painkiller addiction that would lead to a devastating bicycle accident where she nearly died, which she would later incorporate into another of her songs/videos "911", that stylistically is influenced by the film "The Color Of Pomegranates" & the films of directors like Tarsem Singh.

Lol The video is about how she was raped by a record producer and then dropped from her first record label, having a mental breakdown. It then goes into how she had to build herself back up to become “Lady Gaga” - rehearsing dance, dying her hair blonde, packing up and moving out of New York City, and finally to meeting with Interscope Records, her current label and the one who kickstarted her career as a recording artist as you know her now.

She was “back-stabbed” (to put it in simplistic words) by someone in the music industry as well as dropped from her first label. She was determined to make it in the business, that’s why she says, “I did it all over again.” And the rest of the video shows her putting in the work practicing and dancing and also being kind to her colleagues. Being kind is important to her. And she eventually gets a second chance with interscope records.
