Sir, should we obturate at a reading of 00 or 0.5 to 1 mm less than 00 reading?

The latter is generally a safer length (.5-1mm)

And if you can’t get patency?

There are many reasons for not getting patency. It's important to know what the cause is (calcification, curvature, obstructions, long stranding periodontal disease with cemental ingrowth. But generally, poor instrumentation technique is the most common cause. Crown down and chelation can help. The rest is just dexterity in managing tight canals. If there's no lesion at the apex, it may be possible to leave it at that length. Patient should be informed and possibility of Apico in the future discussed. Referral to a specialist is always a good idea to avoid medico-legal complications. Cheers!
