I have no background information, i just saw this clip here. But i am certain that they are not together anymore. He looks spent, tired, exhausted, detached. Severely depressive, to be honest. But what I can say is that there is no love flowing from him to here (anymore). He either enjoyed her being severely obese or he just didn't recover emotionally from her being so obese for all that time.

It’s normal after significant weight loss to have a period where you “let yourself off the hook” and either plateau or gain some back. Alicia did this, but she got a grip on it and got back in the saddle- credit to her. Dr Now talking about Tim…just astonishing! Go Dr N!

I bet Dr Now knows what he's talking about in regard to enabling from the husband. I'd say he has seen it all! Actually, she does need counselling, in order to fend off manipulation. " As long as she loses weight I'm happy" is a bit of a sign. She isn't responsible for his happiness. That's his problem and that's his choice. But I tell you what, she looks and sounds a lot happier herself.

Wow, wow, wow! Dr. Now is giving Tim a blast right in front of him. And look at Tim’s face….like a little boy. Can’t face Sr. Now’s truths. Tim is a manipulator and wants both worlds. He wants to have a thinner wife because it lifts his status up to have an attractive wife, but he also wants her to need him because it means he can do and say what he wants and she can’t leave him. If my husband told my doctor I needed more therapy and answered my doctor’s questions to me, I’d be livid. She just acts like it’s normal, even expected and wanted. It keeps her from being responsible for her own actions as well. This is a dysfunctionally co-dependent relationship. He needs her to need him and she needs him to give her the excuses she needs to fail and sabotage herself. The best thing for both of these people is to not be together. They’re toxic for each other, especially Tim. He needs to do a lot of growing up.

Oh boy, I am surprised that Tim didn’t get really angry and either get up & leave or give the doctor a piece of his mind. I had WLS in 2004, lost all of the weight that I needed to get to goal weight, and hernia repair and eventually regained almost all of my weight. I lost 35 lbs on my own last year and have maintained that loss. It’s stomach surgery, not brain surgery.

When you’re addicted to something you don’t care how you look. Or rather, you care, but it’s not an incentive to stop because your addiction is overwhelming it’s the only thing you care about. I was a binge eater for decades. I was fully aware of what I looked like eating, and it was embarrassing, but it was in no way a deterrent. I just wanted the food and once I had it I felt better, just like an addict getting their fix. Food was the ONLY comfort I had back then.

He is from Iran. It’s hard to learn a 2nd language and get the pronunciation correct. It is probably why he speaks slowly, also, to make sure he is understood. He is also 80 years old, and his body shows the changes associated with aging…
