In the last few years, there has been a rapid development in the use of zirconia or metal free crowns in dentistry. Zirconia crowns are considered to be the best option these days. But what makes these zirconia crowns so popular?

Hello doctor, Can you please give me advise? A few days ago my upper molar was pulled out and near to this tooth I have a tooth with a zirconium crown. When I looked in mirror I found out that front part of this crown on side of my lips which used to be smooth now only has 1/3 of tooth smooth and seems like 2 millimeters thickness of my crown seems to be gone now. So I don't know what to do; is it possible to polish this crown to make smooth again? Or do I have to remove this crown? My tooth under this crown had a root canal and I hear that removing this crown is very difficult and can sometimes damage the tooth; is this correct?

Hi Doc, great video. I've got a problem , can you help me please?. I had 2 gold crowns installed on premolars right side upper jaw 5 wks ago. The gold crowns are 70% gold, rest are various noble metals including 8% copper in the alloy mix. In 2016, I had 3 gold crowns installed left upper jaw. A premolar, & 2 molars. I'm now tasting a copper metalic taste in my mouth. I never had this problem till the recent gold crown installation. I have no amalgam fillings. Both left & right side lower jaw ,I have PFM on my molars. I am in good health, I don't take any type of medication or recreational drugs. My dentist advised it a galvanic reaction between dissimilar metals in my mouth causing the metallic taste. My dentist has recommended removing 2 new gold crown & replacing them with monolithic zirconia crowns. What do you recommend? I look forward to your reply. Regards James in Queensland, Australia.

Yes, it is probably because of the galvanic reaction happening due to the dissimilar metals. Go with your dentists advise, I would suggest the same...replace the 2 gold crowns recently placed with monolithic zirconia. That should hopefully resolve the issue. :)

Currently I am undergoing with RCT for one teeth(6th one) each on both side(lower) . What would you recommend me regarding crown? Should I go for PFM or Zirconia? Looking forward to your reply doctor.

Hello Sanjukta! Both PFM and Zirconia are ok...I would prefer a zirconia crown. There are some factors to consider when deciding between them, Zirconia is more conservative compared to PFM ( The tooth has to be reduced almost 2-2.5mm for PFM but for Zirconia around 1-1.5mm) , also in case of PFM, the porcelain can chip off over time exposing the metal. When it comes to esthetics, both options give you comparable results...However, PFM is much cheaper than zirconia...so, the ultimate decision is yours... hi doctor , a 60 years old male patient had attended my clinic seeking for zirconia bridge to replace his missing lower anterior teeth , he is suffering from diabetic and has a gum resicion of the abutment teeth , the the edentulous span is 4 teeth , can i go for preperation of zirconia bridge ? and what is your opinion about the issue? finally i appreciate your kindness .

You mean as 'layered zirconia _I guess PFZ_' is not 'graditional multilayered' zirconia (KATANA) which is full zirconia and prepared by Cadcam,, is it true?

Greetings Dear Doctor, I am a fellow Doctor (Physician). I was wondering if I could ask you some questions about Zirconia crowns based on your clinical experience. I would highly appreciate it if you could give your point of view on the following: 1.In your clinical opinion are Zirconia crowns for molar tooth a good option? 2. What common drawbacks and disadvantages have you faced clinically among patients with Zirconia molar crowns? 3. Did you clinically see any patients with root fracture of the underlying tooth crowned with a zirconia crown or the wearing of opposing teeth occlusal surfaces? 4. What are other options for molar crowns in molar tooth with rct done with minor crack/fracture in crown area of the tooth? Thank you in advance for your valuable opinion.

Hello Dr. Fatema, Here are my answers to your questions 1) Yes, Zirconia crowns are great for molars...I personally also prefer monolithic zirconia crowns for molars. 2) One thing I sometimes don't like is the esthetics between the monolithic high strength zirconia crowns (3Y TZP) and the natural adjacent teeth, especially when they are more translucent. It is because these zirconia crowns are opaque. But, since they in the posterior region, its generally not much of a concern to the patients...I usually inform them about this before I prepare the tooth. Second thing is the connector dimensions for bridge, not faced any fractures personally, but for 1-2 cases initially, the technicians have sent my cast back saying the bridge would fracture due to inadequate connector height...ever since I have been careful of that. 3) No, in my limited years of practice, I have not seen any root fractures under zirconia crown...I usually give a polished zirconia crown to my patients as per the protocol...never really had any long term follow ups to comment on the wear of opposing teeth. 4) I think it depends on the extent of fracture, if you have sufficient enamel, you can even give an zirconia or LiDiSi onlay or a crownlay...Otherwise you can really give any other crown....metal, PFM, layered zirconia, LiDiSi depending on the case.

Regarding your answer to Question 4....To clarify a bit more.... The minor crack is on the left 1st maxillary molar (occurred after accidental bite on a fragment of hard bone) ... crown area is only involved along the lingual groove and crack is near to the pulp cavity on X-ray so RCT was done (also clinically there were signs of irreversible pulpitis)....otherwise the tooth structure is intact. Based on the above information in your clinical opinion what are some of the best Conservative restoration options for the tooth that are long lasting solutions? Thanks in advance.

​@@fatemamohammadalam965I believe either a metal or zirconia crown would be good....zirconia if you are giving and there is enough enamel present, you could consider bonding it...I think that would be preferable

Hello Priyanka....Translucent Zirconia or Emax (Lithium Disilicate) crowns will be good and natural looking for your front teeth.... With regards to the longevity of the crown, it depends on a lot of factors like the tooth condition, the operators skills and techniques and your oral hygiene measures.

Ceramage crowns a different from zirconia...they are zirconium silicate...good esthetics but the strength is not as good as regular zirconia
