I think it's about her best friend Anthony who died of cancer a few years ago

Anthony and her miscarriage (the Did she answer? part) and all the other people she's lost. She has lost... A lot of people.

I saw it as autobiographical. She's talking about herself and all the death she's had to deal with in her life (including Anthony, one of her other friends, possibly the abortions she's had).

I think it’s meant to be heard-or at least I hear it-on several levels. There are the artists own experiences of literal death and loss, which certain fuel this in a way. But there is also a meditation on the way a life organized around skill and accomplishment can kill relationships, narrow a person in a way that kills openness to alternative possibilities, and block true connection and creativity. So, I see it also as an artist’s recognition of the self-undermining aspects of artistic virtuosity.
