A 42-year-old lady says she is developing wrinkles around her eyes, and wants to know which radiofrequency device - Pellevé or Thermi - would be best for the delicate skin around the eyes.

Dr. Prasad, I am a licensed Esthetician and I try very prominently to research information on modalities I can use on my clients that will give them reasonable results. I need to know the concept of the use of Bipolar RF. Can you explain that to me? BTW your videos are so educational, they really inform exactly what the difference is between what an Esthetician can do as far as results and YOU as a Doctor can do. Which makes for a good explanation to my clients regarding realistic expectations and goals. Thank you!

RF generates collagen by virtue of tissue destruction And micro needling by virtue of perforation allowing vitamin products, prp to generate not just collagen but also elastin And no tissue destruction Therefore wouldn’t microneedling be preferable to RF & other laser treatments ?

Hi @@RK-su4hs , You are mistaken as radiofrequency does not generate collagen by destroying tissue, and the main purpose of microneedling is not to allow other materials to penetrate. Both treatments work on a controlled injury model, where the body responds to injury by generating collagen. I hope this clarifies things for you.

Hi @Dandelion_flight, No, they are not the same as Pelleve is a radiofrequency treatment, and Thermage is high frequency ultrasound (HIFU). I don't use high frequency ultrasound in my practice as they heat too fast, and there is less control over temperature than radiofrequency, or laser devices. I hope this clarifies things for you.

In my understanding Ultherapy is ultrasound, the original before HIFU machines came out. At first I thought Pelleve is just alternate name used with Thermage your videos. I thought maybe Thermi is same as Thermage 😅😅. Does Pelleve work in the same way as some of the machines where it cools the epidermis but heats the deeper layers to help with tightening and collagen reproduction? Thank you.

Hi@@wem-c9f, Sorry, my mistake. Thermage is a radiofrequency device, but I don't use it in my practice. It’s important to remember that the particular device does not determine results, or causes complications, but rather the experience and judgment of the doctor using the device. The device is just a tool, and it’s the doctor’s judgment using the tool that separates great results from ineffectiveness, or potential complications. Hi Geraldine Robinson, Radiofrequency treatment is not something you need to do constantly. Treatments are meant to help you look your best, but treatments do not stop facial aging, nor do they prevent the effects of environmental factors like excessive sun exposure or pollution, or lifestyle factors like diet or alcohol consumption. Hi Nath B, There are too many variables to give a number, such as the area you want treated, your age, the extent of the issue, skin type, other treatments used like PRP, etc. An examination is needed before your treatment plan can be approximated. I hope this helps you.

Depending on the cause, surgery may remove them, or they may need to be camouflaged so they are not so prominently seen. Great video. But I am a little bit confused. I don’t feel like you discussed what radio frequency can do to help under the eyes. I understand that it takes multiple approaches. But I would love to know what radio frequency can do to benefit under the eyes. I have heard it can’t even be used near the eyes. But I see radio frequency, model, micro, needling All over KZbin. Some physicians use the scarlet model. I like to research all I can before I go to positions. Because many of the ones in my area want to market things to me that I personally don’t think would work for me based on my research. Can you please clarify if it is truly safe to use radio frequency, micro, needling, or regular frequency around the eyes. Thank you so much! I meant physicians. Not positions lol. Many physicians here we’ll just try to push you to do the Newest whatever machine they have. Or they will just recommend something that you can’t afford. That’s why I try to do my own research before going so that I am prepared when they recommend something.

Hi @littlebeebs1, The treatments you mention like microneedling, which also include lasers, and radiofrequency are collagen induction therapies, which trigger the body to generate collagen to strengthen the skin. These work by creating a controlled injury to the skin, so the body generates collagen to heal the injury, and strengthen the skin. Radiofrequency, and laser treatments use heat to create an injury, but radiofrequency can be used in darker skin types that are prone to hyperpigmentation, hypopigmentation, and scarring from lasers, and microneedling. Hi Ghofrane E, Skin bunching that appear only when you smile are not actual wrinkles. However, it may be possible to still improve quality and thickness of your lower eyelid skin. Hi Poison Ivy, Treatment can improve quality of the skin, which does last. However, treatment does not stop normal facial aging, nor can it prevent the effects of lifestyle factors like excessive sun exposure, diet, lack of sleep, etc. I hope this clarifies things for you.

Thank you for all the information. My question is what wave is best for Mexican descent. Fitzpatrick 4-5, 42 yrs old minimal wrinkles, no sun damage, minimal post inflammatory hyper pigmentation from hormonal acne. Collage and elastin are good. Hi Maria Garcia, There are several treatments available to help with acne scarring, from radiofrequency, microneedling, subcision, injectable fillers, PRP, and others. I can't say what's best for you until examining your skin and the type of scars you have. Hi stella dalton-kirby, I would have to examine your face before saying this is the right treatment for you, and the ideal temperature if it's the right treatment for you. These treatments do have to be spaced out for more than a week. If you have light skin, then there are more treatment options available for your skin type, such as lasers. Radiofrequency is often used for darker skin types as lasers can cause pigmentation issues in darker skin. I hope this clarifies things for you.

Hi Christopher Lopez, I don't recommend that be attempted as lower eyelid skin, which is the thinnest in the body, will be damaged. It is possible to excise or cauterize prolapsed orbital fat with a laser, but this is done surgically from behind the eyelid. I hope this clarifies things for you.

Hi Michelle Urbano, Since hollow eyes are due to lacking volume underneath the skin, or having eye bags that make the adjacent area look hollow, an invasive procedure is needed to treat behind the skin. PRP is for treating skin quality, not a treatment for hollowing. Hi ANKIT KHENI, Microdermabrasion can help with removing the top layer of skin so a new layer of skin can grow in. If you are having it done professionally, then they would likely use microneedling and not a dermaroller, which doesn't roll so it doesn't unnecessarily stretch the micro holes, and aggressiveness can be controlled by a professional. Hi Sanjay Nagarkatti, It is always important to be conscious of the underlying tissue when doing radiofrequency or any other thermal treatment like lasers because excessive heat and exposure can compromise the essential fat layer below the skin. Hello doctor, today my wife went for hair removal, but on the forearm, underside the skin has come up in welts and is red. Cold compress for past four hours has not subsidef the redness. The attendant was using a new diode machine with pre-programmed settings for skin colour. The plug kept slipping through the treatment and probably on that point the setting changed to darker skin. Will there be any serious damage to underlying tissues. Could it lead to cancer. What are the implications. Kindly reply urgently. Can I buy any medications for her.

Hi Sanjay Nagarkatti, Sorry to hear about what happened to your wife. Although settings can be adjusted for skin color, they don't prevent overexposure by the practitioner. This is why I keep insisting that devices are just tools, and it's the practitioner's experience that matters most. It is highly unlikely to cause cancer, but you will have to consult your physician about managing the symptoms as I would need to perform a physical exam on the skin, and know the details about the procedure before giving exact recommendations for skin rehabilitation. I’ve been binging your videos for a week now and they’re incredible. Thank you so much for all of this information! I was wondering since you said RF stimulates collagen production from your own skin & there’s a limit to how much collagen each person has. Would that mean that using RF in your early twenties would make it so that there is less collagen to work with in your 30s?

Hi Tahseen Reza, You're welcome, and thank you for watching! While the body synthesizes collagen throughout your life, it's the capacity to synthesize collagen as one gets older that declines, which decreases by about 1% a year after age 25. As your body's collagen synthesis is still at its peak in the early 20s, collagen induction treatments are generally not advised at this age. If you are still in your early 20s, I would only advise you keep your skin protected with sunscreen and moisturizer, as well as limit excessive sun exposure to prevent premature aging, but usually not collagen induction treatments. Hi Uma Rajiv Rao, It's covered in the video that radiofrequency can be used on the upper eyelids, but it's not the only option treatment, and a combination of treatments are also a possibility. As treating any issue with unique factors in individual people, a physical examination is needed to determine the best treatment approach, and not just recommending one device for all people. Results are determined by the doctor's use of the tool, or choosing a different method and rate of exposure for your situation, as the tool itself does not determine results. Fifth time failed to scrub and clean my face properly got a slight infection which faded but then I developed an acne after 2 months. I treated it but ever since my skin has become so sensitive. I cannot use any cream. They used to tell me I have resistant skin more than normal but now am not sure anymore. Though my face is proportional and I started it at 30. Now at 35 I feel the facial volume has decreased a lot. Am not sure if it s due to this or aging too.

It was marketed as preventative, for skin rejuvenation and they advised 5 sessions for good results: three the first year and then one for the fourth year and another for the fifth year. Each time they kept saying my skin was resistent and increasing the frequency. Anyway, my face looks normal but I notice it's hallow beneath the eyes more than what I would expect at 35.Usually a session is only 10 to 20 minutes long.

Hi Lore Strahan, Fine lines that appear when you smile, called dynamic lines, can often be improved without surgery using treatments such as Botox, perhaps laser or radiofrequency treatments, and filler is a possibility as well. All I can imagine is that the skin underneath that's being cooked is like a cooked piece of chicken. Does the the body eat away all the cooked skin and replace it with new skin underneath? Or is it just heat up hot enough just for the skin to get a reaction?

Hi Kyle 0, "Cooking" the skin and/or essential fat under the skin is another term for burning, which is not what you want to do. Using thermal devices should be done so there is enough heat to create a controlled injury, so the skin contracts and tightens, and the body recognizes the injury so collagen is stimulated in the area to heal it, and improve skin texture and strength. Hi G&T HD, The point of the video is radiofrequency treatment can be used on darker skin, especially if certain lasers can cause hyper or hypopigmentation. Lasers can also be adjusted for darker skin, but a physical exam is needed to determine the best mode of treatment for you. I hope this clarifies things for you.

Hi jawar Lal, Those are the two primary treatments for number 11 lines, in addition to platelet-rich plasma (PRP) to improve skin quality in that area. PRP is usually applied in conjunction with a cosmetic filler for number 11s. I hope this clarifies things for you.

Hi Just look at the Flowers, Quality care is certainly not cheap. However, I would advise that you have not have any procedure done at all, than to settle for a cheaper procedure which may cost you more in revision work and skin damage. I hope this helps you.
