He forgot smoking, luckily someone reminded him, also didn't ask about drinking and use of recreational drugs, not forget lady of her age I would always confirm pregnancy status if any just to make them aware of the potential abortion. Having said that it is very easy to point out mistakes while lying on your bed. Job well done!

Could she be pregnant & cocp use? Sometimes if they just get a depo injection may forget. Otherwise it was slick, thank you very much! For my osce I also need to say what tests to request after preop assessment if its possible to get that too afterwards that would be really useful!
