lanava53 he was undercover. She...found him...i guess you could say. They slept together. She kept coming back to his undercover place. Guy he was trying to get enough dirt on to arrest got suspicious and kidnapped sam. Basically their illicit affair endangered sams whole undercover gig and so it was either a 3 month suspension or get fully fired. From what i can remember of the end of season 2. This is the gist of it

It wasn't Andy's fault that Sam was exposed, it was Boyd who gave him a boat as a cover background and so he was exposed. What came out of the investigation was that she was suspended because they weren't actually allowed to meet Sam, no matter in what form ​@@g2growcya
