You need to look at the effect caused by 90% narrowing. If he had a heart attack or has chest pain on exertion I suggest he better undergo stent. If he has no such symptoms, I suggest you to get a test like Stress Thallium to decide on whether he benefits from angioplasty-stent or medicines are better
