I'm guessing the voice sounds different to them? Also if someone were to notice that, it would be Nagisa, not Karma, since you don't hear your own voice the same as it sounds to others.. The reason is, your ears sort of block your own voice, so you won't harm your sense of hearing.. (English isn't my first language, so I'm not exactly sure how to put sentences together)

Pointers! **This is just a theory, so please be respectful** At the end of the anime, Zack doesn't just magically come to save Ray. Instead, Zack is executed as planned and Ray cannot bare the thought of him breaking his promise as he swore to god. Ray hallucinates Issac coming to fulfill his promise and stabs herself, the ending being a mere hallucination / dream of hers. This is an anime with a sad ending, in which our two beloved main characters both die and the anime ends. Sad I know- but I believe this was the creator of the anime's intention. To make it sad. However, feel free to make up some other theory where they both stay alive or something :D
