SPOILERS I think he's implying that the Beheaded is actually the King because the first time you encounter the King himself you can see that he's a dead corpse infected by the Malaise, so 250 years would make a lot of sense probably, homever there is no clue of when the Malaise started

Guys im evil so proceed with caution its a spoler The alchemist or the collector is the MAIN VILLAIN. The king got infected by the malaiss a lot of years ago and the collector decided to help transform the king into the blob like the main character, but. U see he f-ed up and accide tly erased all of his memory. The king ended up in the prison. Meanin, the chosen one is the king. Now the queen or the kings wife had fone the same type of transformation but a successful one. I will go into details if this goes popular
