This helped me SO MUCH thank you! I've searched lasers & resurfacing options a few times in my area & always felt totally lost in all of the dermatologists, spas, medspas, plastic surgeon offices who offer all the different procedures which were mostly IPL & microneedling but now I know what to avoid 😊 I found a doctor that's been in practice for 30+ years and checks all the boxes you mentioned to find the right provider. Finally! Someone giving it to me straight. It can be overwhelming to choose what to do and there’s tons of misconceptions and of course, people product pushing with ingredients of unknown efficacy and a fancy name. I cringe when I see people using the at home micro needling devices.

The effort and accuracy I put in my at home procedure, it will never happen in the office. They touch everything with their gloves and then touch my face. I do much better job because I do care about my face much more than any doctor. And I use same professional products.

GUYS dont waste your money on microneedling!!!! I have acne scars that I wanted to get rid of before my wedding, and this place convinced me that microneedling would do the trick and I bought a package. Come to find out my scars are too deep for microneedling and I needed subscision! I ended up going to the scar healing institute in beverly hills. I wish I went there first instead of wasting THOUSANDS on microneedling to still have scars

getting rid of acne scars is very hard....I've done it all even subscision....I actually worked in a plastic surgeon/ dermatology office...it can be improved but never removed..

Microneedling has done amazing things for my skin texture and collagen building. It sounds like you were sadly recommended the wrong treatment for the magnitude of your acne scars.

Extremely helpful video as a rough guideline on what we can bring up to our own board certified dermatologists. I understand the hesitation to give bullet points on what treatments to get because each patient experience is so customizable but it’s a great way to say, “I’m interested in getting Fraxel/Cleae & Brilliant for X, am I good candidate?” when we’re reaching a certain age (or more likely, see certain skin concerns). Sometimes dermatologists don’t recommend something until we specifically mention we’re interested in something and these types of videos help us know what’s out there. I’ve seen comments where people say their broken capillaries have been bothering them and once they see a clip of someone getting V Beam they are surprised and say they had no idea something like this existed! Patient education is so important so we can ask the right questions. I also appreciate videos like this because you pretty much explain the pathway to aging beautifully (whilst still looking natural). It’s some of the things we wished certain celebrities that are aging super well would divulge but they usually don’t (either outright deny work or give tips here and there like breadcrumbs). I don’t think anyone on KZbin is doing what you’re doing and at this depth (most dermatologists on KZbin understandably want to keep things affordable). Thank you! Also love your skincare line.

Thank you for sharing that with me :). Sometimes the best treatments are harder to find and less talked about because its easier for medispas to offer treatments that are much simpler to perform for less highly trained providers. But the downside is patients having suboptimal results and not really seeing a difference in their skin after spending a lot of money. That is why they offer packages etc, largely because people aren't happy with the results but they get tied into getting subsequent treatments because they bought a package. I see it all the time. I like to just increase awareness of these things on KZbin because I encounter it everyday with my patients. They usually end up seeing me after spending thousands of dollars on cheap botox, treatments that made no difference or complications. Then I go on the other side and teach providers in medispas how to use different devices and injectables to find that many of these providers are learning on KZbin how to inject and use these devices so it just motivates me to do these honest but educational videos for you guys. Just so you're informed and see the other side of the darkness in the beauty industry. Thank you for your comment and for watching!

Finally someone saying that IPL can aggravate/cause melasma! I had it done many years ago, initially was great, lots of freckles and sun damage sloughed off, then BAM melasma all over. Even just the summer heat will make it darken. They told me that wasn't possible. 🙄 I have to wear so much mineral sunscreen just to keep the patches from being so dark. Would fraxel help this?

These 5 are not described as what to avoid, rather how you could tailor your budget... 1.) Beware of packages- *(except end sculpt, where you need multiple sessions)* otherwise you should notice results from one treatment or be able to switch things up if there aren't results. 2.)Microneedling- issues with scarring and infection, less than optimal, practitioners can be poorly trained 3.) Peels (can keep you looking fresh, but not a must have) 4.) Facials- can help but if you are getting procedures like Clear and Brilliant, they might not be necessary, so pick and choose. 3.)Thermage/ tightening devices- only once every 5-10 years but Ultherapy is not worth it. *(I personally agree with this, it was a total waste!) *Over 50 is more important to budget for. 5.) Focus less on fillers once lip and cheek volume/facial symmetry are addressed. * Use Botox every 3, 6, or 9 mo. (dependent on activity of area treated) * Under eye Fraxel 2x a year * Budget 1K-5K+ mainly for: lasers (resurfacing once a year), skincare with actives, mineral SPF This is just my summary, but defiantly watch the video and make your decisions on what she said.😊

Love this! 26:44 is me 😂 I agree with everything you’ve said. I started following you and never went back to microneedling. I have my sinking fund where I save up for a laser for my birthday in the winter and keep using my good skincare, sunscreen, etc all year round.

Could you also make a video about different lasers (differences, pros and cons)? I'm still young and have a lot of fat and collagen in my face. I would be afraid that all of this would ''melt away'' under the laser and I would end up looking old. I've observed this with some women. From how old would you recommend which laser (generally, of course everyone is different)? All I've been doing for years now is using tretinoin, 20% AHA peel and daily sun protection.

Retin A overtime and starting early is very effective. Epidermal growth factors/PRP I find extremely important after 40. Also, minerals taken internally that support skin is equally important. Copper and zinc are a must. A high protein diet to support tissue and muscle within the body is a necessity, along with daily sunscreen.

Love your channel so much! I absolutely LOVE the Vbeam laser and I completely agree it does wonders for skin, I see so many rejuvenating benefits from this laser. I've done treatments since my late 20s (I'm 49 now) and I estimate I've had about 12 treatments over the years. So many offices are pushing the Broad Band Light treatment as anti-aging, and a few years ago I asked you about it on Instagram and you told me the exact same thing, that VBeam was much more elegant and precise. Thank you for that advice, my skin truly continues to look great!

Great!! Thank you for sharing!!! Yes BBL and IPL are always pushed and more available because there are a less elegant and lower tear device. Not truly a laser by definition. When I worked in laser institute with nearly 100 devices the BBl and IPL devices collected dust. None of us ever used them because we had all the lasers which all gave next level results!

Love your videos ..your skin is gorgeous...just like a porcelain doll. I just started looking into tightening procedures at 53 and unfortunately my Dermatologist told me I could not do any kind of procedures because I have hardware in my jaw from a bilateral split osteomy..I:m so disappointed..! Have you ever worked on any with hardware in their jaw ....again love all your knowledge you share ..❤

IPL in the late 90s for me. Plus glycolic peels. Then I took a break for a few years while I had kids. Came back to treatments with thermage and fraxel. Just got Botox for the first time last week and turning 47 this week. Got a few issues to catch up on (texture and redness) but overall I think the early investment in laser treatments helped delay a lot of the normal ageing signs.

How come PDO threads are not mentioned??? I understated some Drs are not supportive of it due to the cost vs longevity of the procedure, but when you are at an age that a surgical facelift is really not the best option, PDOs come in. Since I discovered PDOs, I kept them in my annual procedure sequence for twice a year along with Botox, Scupltra, Resytalene and some form of laser (sometimes Venus Viva, sometimes Thermage, will try Fraxel soon but shooting for a fully ablative CO2 next). Also home treatment of microneedling (my least favorite), IPL, chemical peels and red light therapy support eachother. The total of my annual budget is about $9k in my 40s now.

It's funny you mention microneedling and bad results: I actually did this as my second treatment to microcurrent. I got way better results doing microneedling at 1.5 depth in any place that could handle it, and 1 depth in more delicate areas sometimes 0.75 like under eyes, than expected. I mean it took away my jowling and improved my neck laxity. I did not pair it with RF even though my NP who did my microneedling does have a Thermage device. I'm totally terrified to do Thermage and she reassured me her device is the newer one and has safety features so she will not accidently melt my fat. She also does not do treatments she herself has not done on herself and is totally comfortable with. So, in a year or so, I'm 38, I may do a RF Microcurrent combination. Basically whenever I feel I need a touch up. My microneedling was face, and neck for 275$ so it isn't cheap but I would say it's not totally a waste. I just think it depends on the person and their skin and level of aging, and most importantly the user and depth they got it at. 1.5 depth is painful even with numbing cream and that's the sweet spot for producing collagen. My aesthetics provider said most of her clients cannot do the 1.5 mm as it's too painful and because they cannot go that deep with the device they do not get the best results in their cheeks/jowl area. I have a whole back tattoo and I will say, it's painful even with numbing cream. Bearable, but painful. I heal really fast as well. In 2 days no more red or any sign I got anything done. 4 days I'm fine and good to go. I do hydrate the F out of my skin though. Now I will say, she's a well educated provider so perhaps that's why I got the results I had hoped for, but did not expect. Her and I discussed at home microneedling and she said to never go that route because most devices that are not medical grade board certified cause micro tearing. Further, you would need to go at a 1.5 mm depth in most areas to really boost the collagen production, and you run the risk of infection. So overall doing it at home is risky and less effective as most people are doing it properly to increase collagen production. I understand why you do not like micro needling though because of all the ads to do it at home. NO ONE should do this at home. It's very dangerous to do this at home and you really need a medical grade machine with the proper depth to get good results.

I've been getting Botox for Chronic Migraine every 12 weeks for about 11 years. I have excellent reduction in severity of pain and the bonus of a pretty smooth forehead at age 60😊- just in case it helps anyone who thinks it's not been studied/used over time. Board certified neurologist does mine-31 injections around head, face and neck

@@pionus3651 Do you know any of these people personally? Read the real literature (PubMed, etc,). It's the most widely studied treatment of its kind. !6 yrs, 4500+ peer reviewed published studies. I have a board certified neurologist administering mine. Have for 10 years. Where do you think it's migrating? It's out of your system in 12 weeks (unfortunately). Yeah if you have a hack pumping it into your face, you might get a droopy eye lid. Get a better provider. For some of us it makes the difference in having the will to live

I had IPL 20 yrs ago for broken capillaries and they’ve only now begun to return though I do have some rosacea type redness. Torn between Vbeam and good ol IPL.

Have you heard of Daxxify? The only thing that really worries me with Botox now is that it uses human blood (Albumin). Although it may be purified, they can't get prions out. They MAY contaminate it. But the major concern is the MRNA spike protein contamination. I'd be interested to hear what you think of Daxxify. It doesn't contain either human blood or cow blood and last twice as long. Have you heard of it and will you be offering it?

I did IPL and it did have an effect on my pigmentation but it took A TON of sessions, and in retrospect I should have just done a Fraxel. Planning on doing a real laser in the fall though ☺️

Is there a point where NOTHING will help? I am 70…have saggy skin and wrinkles due to the laxity mostly around mouth ie marionette lines and nasa labial folds. Is my only option a face lift?? Thank you doc. I love all your passionate, informative videos!

Hope this hasn't been asked yet, I am considering Cool Peel...just finished my last of 3 rf, first 2 were Morpheus 8, last was Pixel8 and I sure wish I had seen this video beforehand as I think I wasted $2400! Saw some minor improvement but still 😳 I'm 64 with all the normal aging concerns unfortunately. Thank you so much for educating us! ❤

The derm where I had pico laser told me she did not see a difference with cool peel even when really analizing in detail. The clinic offers cool peel, but she said she does not recommend it.

I love your videos. I am 41 and suffered from acne when I was a teenager. I was on accutane and it was a miracle for me. I always fear of it coming back so I take spirolatune. When I turned 30 I started microneedling I felt like it was a joke. It didn't even do anything for me and then started doing Morpheus with prp. At 38 I started seeing a dermatologist and left the plastic surgeon I was initially seeing. I have a good skincare regimen btw. I did several CO2 and two halos. They are both rough but I do use the alastin necktar to speed up the healing process. the halo heat part is the worst part but I noticed for me the halo doesn't do anything for me to be honest. My skin has definitely changed for the better of course but I still have some imperfections in the cheek area. my question is I'd like to get get facials but the hydra facial made me break out a little bit in the chin area so I don't want to do it again. What kind of facial would u recommend. I did ask my dermatologist but she says since I do lasers twice a year she thinks it's not necessary. I wanted to hear your opinion. Sorry for the typos btw. Lol

I'd do a hydrafacial with oxygen infusion. I'd also continue with Fraxel restore for texture if you can

Is there a way to know if the doc is injecting watered down botox, are they required to tell you? Or do you have to assume it is watered down if the price is too good to be true? Also, do you feel that Dysport is just as good as Botox, or is Botox better?

Curious if your microneedling aversion also applies to at home devices? Not the "dr pen" types, but more professional/accurate ones like the Derminator 2. There is a lot of science showing the efficacy of regular microneedling, no?

Thx doc, however i will testify that micro-needling+ PRP really works!! It smooths the skin and stretch mark like scars on face too! My doc injected prp in the scar area...took 6 months post 4 treatments but got smooth skin. I feel its more organic than lasers that heat up skin artificially!! I stay away from lasers.

Vit c works well in the morning as it protects the skin(under spf), retinol at night because it can photosensitise your skin

Another factor to take into account regarding Botox prices is where you’re going. Some practices charge more than others for the same quality Botox. Some practices that buy larger quantities can charge less for the same quality.

As always, I’m so grateful for your time and knowledge! I had 3 VBeam treatments done in the last 5 months for rosacea and broken capillaries. I have more redness and problems with flushing than ever. Have you ever seen that, and do you have any guesses as to why? So frustrating!!

Oh no really? VBeam should always make flushing better not worse. Perhaps incorrect settings were used? Theres always ExcelV and KTP as maybe yor redness with respond better to those wavelengths?

You know since you suggested that I’m going to run them past you 😂 I was driving 3 hours one way to this girl :-/ They are supposed to have a good reputation in that area, but how do you really know? Despite there being money in our area, and people moving here in droves from up north, we still have awful health practitioners here in coastal SC. We’ve always had to go to other areas for most things, so I’m willing. I just have no idea where. If it could have been one and done I’d have been out there with you in a heart beat!

@@DrStefaniKappel I hope this means something to you.. She said it’s a 10mm hand piece, and the wavelength was 600 on VBeam Prima. Not sure if this is relevant, but I’m just a bit more pale than you are, with a strong pink under cast (even in areas that don’t appear to be effected by rosacea).

Hi Dr. Kappel. Thanks so much for all of your time. I love your channel! I was wondering if you can tell me how much time prior to a Fraxel treatment a patient needs to discontinue Tretinoin application. It's been my experience that Tretinoin makes one's skin hyper-reactive to any "abrasive" treatment. Ex: even getting something as non-invasive as a facial can cause significant redness and reaction due to the skin's sensitivity from the Tretinoin. Again, thanks so much for the amazing content.

I did the Ultherapy yesterday and scheduled in one month for Radio Frequency Microneedling with Erbium Laser Resurfacing… I’m nervous - seeing a lot of different reviews 😮if you see this please let me know if I should ! Hey Doc , I’m a man who just turned 28 and I was trying Castor oil with micro needling at home . I had an allergic reaction that created a Nasolabial fold on one side of my face and under eye lines directly above my cheeks . I had often been praised for having good skin and looking younger and this really hurt my skin. Should I get PRF to help or Fraxel or some other laser ? I’m a very healthy man with a. Clean diet and I workout consistently so not sure if PRF is the better option

I’m 45 and do Fraxel once a year. At La Jolla Plastic Surgery and Dermatology. What are your thoughts on radio frequency? I have tattoos on my neck and chest, and been told by someone over the years that RF may be safe over tattoos. Although I’ve never tried it. (I’m removing half of my neck tats anyway.) What are your cruelty free post procedure recommendations for products? I’ve been using IS Clinical Shield with Vanicream gentle cleanser. I live in San Diego. I think I’m going to make an appointment, drive up to your office in Newport and see you for my next Fraxel.

I purchased a package of 3 clear lifts lasers. I had my first and I’m not to happy with the results. It’s been just over two weeks now and my skin looks very dry. Had one prior in my home town several weeks before and I believe it looked much better. Unfortunately that esthetician didn’t work out for me so I traveled 2hrs away to another. I did mention to my recent esthetician that my skin appeared dry and cakey and she suggested a Aqua Gold Fine Touch with Botox and PRP to help. She also recommended maybe opus in place of my second procedure..of course at a higher cost. Would appreciate thoughts on all this. So confused and overwhelmed! Thank you!!
