For 3rd stage - Everyone else giving Earth based answers, but: Starship is sized specifically for Mars. The upper stage needs to be able to do direct launches to Earth, and have the appropriate fuel tanks for that launch, and landing on Earth upon return. Furthermore, it has to do so with fuel made on Mars. And all elements need to be reusable. All design decisions are made around these elements. Everything else gets buiIt around these parameters. Fully fueled, the Starship can do injection to Mars from Earth orbit. This leaves two routes here: either the booster stack is large enough to deliver Starship fully fueled to orbit, or you do orbital refueling. If you deliver it fully fueled, you need to land some booster elements that are now doing orbital re-entry. It's either a follow-up first stage you need to build that permits orbital re-entry, or you need a middle stage that permits orbital re-entry. This significantly complicates the design problem (and is the reason F9 stage 2 isn't reused). Orbital refueling solves this by allowing lower energy first stage to be landed conventionally (it feels funny to say that). I think they will add the 3rd stage , if we see the past of spacex they always choose simplicity and efficiency, if adding 3rd stage is more effective than going with 2nd stage then they will definitely do that , only we can do just wait and watch
