Aortic regurgitation / Aortic insufficiency : Etiology , Pathology , pathophysiology , clinical features , Diagnosis and Treatment
Aortic regurgitation (AR) is a valvular heart disease characterized by incomplete closure of the aortic valve that leads to reflux of blood from the aorta into the left ventricle (LV) during diastole. Aortic regurgitation may be acute (occurring primarily after bacterial endocarditis or aortic dissection) or chronic (due to congenital bicuspid valve or rheumatic fever) and may be caused by valvular disease or an abnormality of the aorta. In most cases, acute AR leads to rapid deterioration of LV function with subsequent pulmonary edema and cardiac decompensation. Frequently, chronic AR may remain compensated for a long period of time, becoming symptomatic only when left heart failure develops. Auscultation reveals an S3 and a high-pitched, decrescendo early diastolic murmur. Another characteristic diagnostic finding is widened pulse pressure. Echocardiography is the most important diagnostic tool, both for confirming the diagnosis and determining the severity of disease. In asymptomatic patients, conservative treatment consists of symptom management and physical activity as tolerated. However, symptomatic patients or those with severely reduced LV function should undergo surgical aortic valve replacement

I nearly died of MRSA at 36. Was diagnosed insufficient last year after trying to pick up jogging in 2021 and trying again last spring. It's like hitting a brick wall-your breaths no longer work the second your heart hits maximum. Today my symptoms are suddenly terrible. I'm only 46. It usually takes a decade to become symptomatic. Pretty scared. Couldn't even pick up rocks in the garden today. Yikes.😢

Thank you for this video! Can the free clinic do the test for this? I was leaning over and noticed that my hair was moving like a pulse. It scared me to know that my head was shaking like that then I noticed on my right side on my neck in the front pulsating spot.

Sir, One of my relative has this prominent pulsation at suprastrenal notch. She has been suffering from intense neck pain, shoulders pain as well since last 7 months. Also there is pounding sensation in head. And she also complaints of irregular heartbeat. What it could be due to?

She needs a cardiologist STAT. I have aortic insufficiency that just suddenly got worse today. I feel like I'm hanging upside down. Breathless at nothing, dang near pass out when getting up, worsening cough (my lungs are A+) Feels like being strangled, get whoosh with pulse in your head. So something is up with her aorta big time.

Why does an increased Left Ventricle End Diastolic Pressure (LVEDP) cause pulmonary congestion? I thought the dyspnea mainly developed due to less reserved pressure and power of the left ventricle to maintain the cardiac output, especially during exercise.

Because not enough blood is exiting the heart. The heart has to empty in order to create enough inflow at rest. It can leave too much blood in all parts of the system, and symptoms show up when the larger right ventricle can no longer compensate for the left.

So in asymptomatic patients what is the management once discovered ?
@andreaberryman5354
@andreaberryman5354 Жыл бұрын
I am on a screening schedule for monitoring. Mine is moderate, but my symptoms just suddenly got worse today, so that is when you notify your cardiologist right away. I'm not up for a screening yet, but you know your symptoms and always make a mental note and monitor them yourself, basically.

Why decreased diastolic BP in severe AR?
@andreaberryman5354
@andreaberryman5354 Жыл бұрын
Believe it is due to too much blood present in the heart at rest, rather than in the body where it should be.
