Well done, as usual. As for me, i tend to use a simple rule to decide what to do in cases like this. If I thought about doing extracapsular, i will go for it.

Doing phaco or even SICS is not free from risk in such cases. We must be free from Medico-Legal Issues while operating such extremely complicated cases. Our AIOS Office bearers and other Senior Surgeons must raise this issue.

This patient's eye (cornea) has condition named crocodile shagreen which has decreased the visibillty (decrease in contrast along with decrease in depth perception), but still the surgery has been done very well. Its always great to see a cataract surgery being done by Deepak Megur Sir. Thankyou sir.

Great case as always! What is your thinking behind using the pulse torsional phaco? Is that better than continuous phaco with controlling delivery through the foot pedal?

CDE 79.4 is too much I saw very nice videos for you doing such a case with chopping technique with much less energy Also I saw you deviding with a nice way using longitudinal power and high vacuum to hold a quadrant and divide with much less stress I really appreciate your great skills and the way you understand and explaine to us your steps
