Sharing personal real life experience: I'm a doctor. Once my ill mother came HOME after receiving a whole blood transfusion. Suddenly she felt very restless. Her BP was 70/30 mmhg. I was expecting the worse. I had iv fluids and emergency meds at home (since mother was battling cancer). I rushed iv fluids, but blood pressure wasn't coming up. I was not knowing exact adrenaline drip rate or doses. In extreme desperation, i added 1 ampule adrenaline in 500ml normal saline, started it slowly. I was still expecting the worse. But fortunately and gradually my mother improved through the night, and felt better, and improved in a couple of days. It felt like a miracle. Sometimes we have to do the best with what we have got. I am no way advocating these practices, just sharing my very personal experience. (Had given IM adrenaline initially also, shock was not resolving. Shock started after 6 hours of blood transfusion completion).

we can draw 9 ml dilution in a 10 ml syringe and next add 1mg/ml adrenaline from the ampoule.total is 10ml. which is an IV route for adult cardiac dose on 1:10,000 proportion.

why not fill the 10ml syringe with 9ml saline and add the 1mg/ml totalling 10ml? please correct my calculation. thanks.

I agree with mel. 1:10,000 is not equal to 1:11,000. You might underdose the patient because epi is overly diluted.

You have to dilute 1:1000 to 1:10000, you don't do that by multiplying by 9, you have to multiply it with 10 so you get 1:10 000, if you multiply by 9 (dilute with 9 ml) you'll get adrenaline solution of 1:9000 so you'll have 1 mg in 9 ml in a syringe that contains 10 ml, it doesn't matter that the syringe contains 11 ml, because the concentration of adrenaline in that syringe is 1:10 000. You just give as much as the patient need. Hope that clears it up! :D But if you fill it up with 9 ml and add adrenaline 1 ml, it won't make a big difference, the difference in dosage is negligible.

It used to directly intra muscular in anaphylaxis. Concentration is 1:1000.

Why not use an insulin syringe and take 0.1ml of adrenaline and 0.9ml of saline? Pushing 0.5 ml of it would be easier.
