actually with this new update is very useful to cleanse every totem because they are now perks that glow after the doors are available so you don't know which one is the right one until reaching the end of the game

It's not the killers heartbeat it's the characters heartbeat it gets louder because if your character sees the killer and the killer is coming closer the heartbeat will get louder and louder
