Question, do you happen to know if let's say a med mal case settles for $4Million and the malpractice insurance covers you only for $1million. Who forks up the rest of $3 million? If it was in hospital setting, does the hospital pay the $3 million or can do you still have to pay over time?

Are there safe guards I should have to protect one practice location's revenue from being grabbed by an incident at another location? Should each location be under it's own LLC like separating each rental property? Will using employees across multiple sites pierce the corporate veil?

Yes, there are strategies that you can implement. The whole goal with asset protection is to create separation between value and liability. This can be accomplished by having each property held in a separate entity as well as each location operating as a separate entity.
