We actually filmed the procedure twice. So he really kept it together. But that goes to show the importance of good topicalisation with local anaesthetic.

I have never come across regional block for airway topicilzation, but I have had older colleagues demonstrate injecting 1 ml local with the thin needle through the crico-thyroid membrane. But personally I do not find that it gives any major benefit.

@@InterAnest Use a sword swallowing technique most of the tools used for the airway were invented by sword swallowers. Basically have the patient look at the ceiling so the tube can be put straight down the throat with no gagging. I have done a endoscopy like this and works very very well.

You are of course correct. Somehow fiberoptic intubation (“fibertuba” in Swedish) is so ingrained in our colloquial that this mistake slipped by five consultants involved in this production. :) I’ll change the title.

Technically, yes, but this technique is usually reserved for patients where general anesthesia to the level required to be able to manipulate the airway is deemed risky, due to this technique being more time-consuming than the regular approach. 1) Sedating / inducing full general anesthesia in someone with a known (or suspected) difficult airway can have lethal consequences if airway management fails 2) Sedating / inducing full general anesthesia to the level required to manipulate the airway can be dangerous in patients suffering from circulatory failure (eg septic shock etc) 3) Sedating / inducing full general anesthesia and inducing apnea in a patient that is sensitive to rises in pCO2 level (eg RV failure) may be risky, which is why this technique allows the patient to spontaneously breathe throughout and after the procedure 4) When inducing general anesthesia, the patient is usually placed in the supine position to accomodate bag-valve mask ventilation if needed (or while awaiting onset of paralytic). As GA and pharmacological relaxation reduces muscular tone, it may be tricky to use this fiberoptic technique in anesthetized patients due to the airway not being open and passage with the scope being tricky. Additionally, something as simple as fog or saliva on the camera can obstruct the visibility of the anatomical landmarks and make the procedure hard to perform.

Nothing. The airflow from your lungs passes directly from your trachea into the tube and cannot be manipulated by your glottis, pharynx or oral cavity. You would just mouth the words.

Gosh i hated having tubes down my nose. Especially the giant camera. It's such a weird feeling, It's really hard to describe. Like imagine a cold metal Tube touching your oesophagus and being unable to clear it, unable to clear your throat. Or swallow to get rid of it. Don't be scared if you're getting it done though. It's irritating sure but painless.
