The only time I recall experiencing predictable fear and paranoia from a game environment was in World of Warcraft, in the ocean, on the route from Westfall to Menethil Harbor. Back when you had to take a long and dangerous route to get from one newbie land to the other, and couldn't easily boat there, it was either a jaunt through level 30 areas at length, or a long and boring swim and the only real danger was in the harbor itself, which was reasonably easy to navigate. So that's a giant stretch of open water along the coast. And there was no reason for there to be content there (although a sort of easter-egg abandoned cabin did actually exist in that area). I knew for a fact that there were zero threats to me: No monsters in the area, no gankers stupid enough to be running around where players usually didn't go. I knew that nothing would hurt my in-game character and that even if it hurt my in-game character it wouldn't hurt my out-game self. No reason to feel weird. But man, did that ever up the adrenaline response of my body. It felt like I was out there, exposed, ready to be attacked from any angle. It was a primal fear situation for me and I had that reaction repeatedly. I have no idea why.
