I haven’t even worked since finishing school, taking care of my own medical needs, and now have an interview in L&D next week! I’ve never seen, set up, nothing for one! Talk about nerve wracking for me. Thank you for the videos.

Very nice to see how others set up. I usually put my suction tubing, bovie, two laps and two allises in the placenta bucket and the doctor and PA helps throw them off. The bovie usually is attached with an allis closer to the doctor. I would much rather have it closer to be for count reasons.

there are a few basic things that you forgot. I would hate for others to get into trouble for not having certain things done correctly with their setups. There needs to be two towels down on the mayo stand. Does not matter what case you are doing... there always needs to be two. You also have no towels put down on your back table. This is critical when having sharps and is used as a barrier. Not sure why a doctor would want warm saline for the uterus. They need a cold saline lap to wrap around the uterus to make it contract. The only time you would use warm saline is to irrigate the gutters (which they have found to be unnecessary), irrigating after fascia is closed or when cleaning off the patient before applying sterile dressing. Tech also need to be careful with having equipment behind them. I would suggest to put your large basin up on your back table so that you dont have to leave the field to retrieve them. And if those are countable sponges under the basin(which i would guess they would be since everything is counted) they need to be visible to your circulating nurse. You, as a tech, need to make sure everything is at arms reach so that you can concentrate more on the surgery and not going away from the patient to grab things for the doctor. You also need to make sure that your sharps are close to you especially when you have med students or residents who are not familiar with surgery. They tend to grab things from your stand (and you have the right to tell them not to) You dont want them getting cut or poked with needles. I keep my sharps on my back table for this reason. They are on the edge of the back table closest to me. Another great reason to keep everything close to you is when it is just you and the doctor. You cant stop retracting for them and go get another instrument or another lap. I have been doing L&D for 19 years now and have got everything down to a science to save time and be efficient. I will try and get some pics of my back table and mayo stand for everyone to see and compare. Remember there is no right or wrong way to set up...just sterile and not sterile. But certain things must be done to keep the sterility of the case.

also I just wanted to say every video that you do please tell as much detail as possible for those of us who don't exactly know why something is done or the name of all of the instruments or I know that when you've been doing this for a while that you you know everything but people often forget that the people trying to learn don't know it yet thank you so much. Also, I hope in the future you wi point at each instrument an say the name and what we are using it for in the case. Thank you so much! Shannon

WOW. Your setup contains a LOT of instruments! Our section trays come with 35 instruments. I'm curious as to how many are in that tray. I'm still fairly new to the OB OR but so far I'm loving it. Had my first crash section the other day thankfully with a good outcome! I look forward to watching more videos on your channel!

Jonathan Sidwell, out c section trays didn't have that many instruments. Always load your kb first. For a stat section, that's all the doc will need to get that baby out in a pinchl

@@melissaj.8071 Knife blade. Before surgery you have to do a count between the nurse and scrub. You count instruments, 4x4s, laps (the white cloths on the table,) kbs and atras (suture.) The patient is often awake to hear the initial count. We use the words atras and kbs for the patient's benefit. They're already nervous and hearing those words and realizing that the knife blade is going to cut into them can just add to their anxiety.

I lay out my laps in rows of five so that it’s easier to count and the nurse can see the blue strips. I would be afraid of losing them on the floor piled up like that. Just a tip.
