All training organisations that offer the diploma should have this as a minimum setup

Maybe she's only human and made an honest mistake. That's the point of having these dummies so students can practice with them and make mistakes without putting someones life at risk. Having said I doubt that this mistake would put someone's life at risk, but they need to make accurate measurements so a doctor can treat the patient properly, I think it was a minor mistake.
