Guts is actually a Hellhound, a black furred red eyed demonic hunting hound appearing in several mythologies. Hunts down evil spirits and brings the souls of the dead into the under. In some versions it's an omen of death, in others a protection spirit. Those descriptions suits to Guts, since he's an Anti-Hero of sorts and I also noticed that this design reference is also seen in the Golden Age Arc, when Guts sword sheaths appeared to be demonic tails in battle.

I've always believed that the lyric "my brother" is a reference to either Guts OR Griffith. That they together would be unstoppable yet divided by design they remain.
