Thank you for lovely presentation... I have a question, in M Alkalosis, some physicians advise to calculate corrected potassium level and adjust if Corrected K is low. Otherwise Patient may has risk for iatrogenic Hyper K after Alkalosis correction. Can you explain more why we have to replenish electrolytes before Alkalosis correction ?

Thanks for reaching out! I haven't heard of correcting potassium level! For what? We may have spurious hypokalemia from lab errors, severe leukocytosis, or if a dose of insulin given right before drawing the blood sample! In such a case, we repeat K level. Hypokalemia can cause metabolic alkalosis through a complex mechanism but eventual leads to HCO3 renal absorption and it must be corrected to switch off the excess renal HCO3 absorption. Hypokalemia can be difficult to correct if hypomagnesemia is present and must be corrected to correct hypokalemia.
