My PT referred by my surgeon never got scheduled after 12 was post op. I am 76 Left TKR. So I went looking for videos to help. And thank goodness I found you. I saw my surgeon at 8 weeks and he was thrilled with my progress. And he wrote another referral and the system still let me down. So I keep looking and doing your videos. I can not thank you enough!!!! Nancy Q

Thank you for providing this video. I just returned from therapy very frustrated and upset. I've done all that they have asked - full sets of exercises twice a day at about an hour a session. My surgery was exactly a month ago. I'm at zero extension and 107 flexion. I'm walking unaided and with only over the counter pain meds. The last two sessions, Monday and today, they were doing the measurement which I have come to dread. Which speaks to your point about the brain protecting your body. They want improvement every session so they push as I pull. The last two days my knee has locked up and I can't move it in either direction until the muscle relaxes or the cramp goes away. Today they told me I wasn't working hard enough at home and needed to push into the pain (not just discomfort) and hold it there for 10 seconds. I'm writing because I don't know how to explain to them that my yelling is scaring their other clients, I have some relief for a few hours but then I can hardly walk in the morning until things loosen up. My nervous system is on overdrive and being chastised is not the way to motivate me. Thank you for letting me vent. Any advice would be greatly appreciated. Joseph

I truly admire your position on rehab. I was making great headway in therapy, as I do it two to three times on my own. But when I went to rehab, the attendant really pushed against my knee to get to a 125 degree bend. I was already at 117 at 4 weeks and making getting better daily. But instead of letting me bend slowly and loosen my knee, he just started pushing and pushing. It was not necessary for him to do that. I would have been able to do it in a couple of minutes on my own. The mentality to push and push is out of place. I can understand if I was not making any headway, but I was at 110 in my first week and improving steadily. Thanks for your common sense approach.

You are totally right Myron. It stinks that so few patients get the opportunity to choose their therapist before therapy. I wish patients could interview their therapist or at least see videos like this produced by their therapist to get an idea about the therapists mindset, personality, and treatment philosophy. I am sure that therapist is trying hard to help every patient succeed, but we are respond differently to different treatments and what works for you might not work for me. How are things coming now? 117 at 4 weeks is amazing. I have patients struggling to get past 90 at 4 weeks. You are doing great!

@@totaltherapysolutions I am doing well, but need to get over some atrophy from being in a wheel chair for over 2 months, waiting for the surgery. I had a traumatic dislocated, fx. to the left knee, after a tree fell on it. Have foot drop due to the trauma and that is what is more annoying lol. The knee is doing well, as I have been doing physical therapy on my own 2-3 times daily during the two months before the surgery and 2 daily after the surgery. I truly like your approach and ideology. I know that my therapist wants the best for me, but their approach seems one track. Thanks for your great videos. It truly is inspiring to see them.

Thank you for this video. I am almost 8 weeks post op for a ruptured Patella Tendon reconstruction and I had a late start to PT and have been pushing really hard (maybe too hard) on range of motion. I tried this today and it has been a total game changer so far. It is comfortable and easy and really helps me to identify which muscles are limiting my range of motion and focus on massaging and stretching those. I have also noticed a reduction in swelling in my knee.

I am so grateful for this post. My son had a tibea break in his growth plate Christmas day...he was splinted bent until surgery day (January 13, he was casted until late February. Then, he went back April 13 for manipulation under anastesia , orthoscopic "clean up" and pin removal. We have been at a loss, because he lost 5degrees on straight in and 10 bending literally over night. His quad has fired like that with just about every movement. His doctor has been less than patient with this process and quite frankly got so tough with him that he was coming off the table.... This is a God send. I know this is slow.. I know.forcing is not the answer, but have yet to see a solution for tha "Muscle fighting" until now.

I was told in rehab that after 6 weeks getting more "bend" in your knee is unlikely. I'm at 6 weeks. Any comments? I'm at 112 and they wanted to get me to 120 this next session. I quit rehab with them. Last time I was very stiff from a trip and they pushed (literally) me from 103 to 112. It was very painful. I live in a small town so I've started on my own. I can't believe I let them do it because this is my 2nd T KR, but it was more difficult to get the "bend" I wanted this time. Thanks Tony for videos and input.

This was exactly what I needed to see. I had a patella fracture with surgery and 5-week leg immobilization. I am into 3.5 weeks after removing the brace and stagnate around 55-60 degrees of flexion. I have been very diligent at doing many things I was told but I didn’t receive the best PT advice so far. What you said is exactly true and confirmed what my Mom’s PT told me over the phone yesterday. We focus on building the quad, but having a relaxed quad is key to get flexion and therefore we must build up the antagonistic muscles too. And if we feel pain and swollen after the effort (as was the case this week with my PT), it is not right. In fact, my knee was nearly blocked for 36 hours afterward. Finally, your suggested practice position is great and makes sense. Way superior to the wall slides. Thank you! Feel free to let me know by private message if I should be scared of just 60 degrees of flexion 3.5 weeks after removing the brace or 8 weeks after the accident. Any further advice welcome. Honestly, I have been very depressed and scared lately by the lack of progress. I am the surgeon tomorrow morning (first time since the brace release) and I am scared as what he is gonna say. I think he was targeting 90 degrees…

I starter bending at week 3, started at 30, added 10 a week. At week 7/8 doc told me to start trying to add up to 30 a week. After 1 week I'm at about 95 degrees but now feels kinda stuck. I'm probably expecting it to go too fast. Ima try this method. Makes sense cause bending feels easier with that knee flex brace that supports your thigh and calf muscles while you bend the knee

Your videos are great, really glad I found you. Almost 4 weeks on a TKR, had extreme swelling from the start , thought my skin was going to burst ,went all the way down into my foot so bad that it felt like someone had wrapped wire around the base my toes and then twisted the wire super tight. Used Ketorolac for3 days and did more elevating with ice and that helped a lot, also got a pair of compression socks and that really helps keep the swelling out of my lower leg. I still have more swelling than most people and when I go to do knee bends of any kind. , my knee is super tight, especially on top of the knee and thigh, just super tight .Is this at all normal? I was on a walker for 2 ½ weeks. and still using a cane , not putting much weight on the can now about 10 pounds, Any time I rehab or use my knee the swelling and heat gets worse, 100 degrees. I was PT on Friday and had 96 degrees rom, hoping for more on Monday, as Have been trying hard at home, but when I work it , it just seems to almost get worse, but I have to keep at it don't I? Pt tells me I am doing fine ,bu t I wonder if they are telling me the truth, I just can't get rid of this swelling and extreme tightness. Doc said next week I can do pool therapy in the mineral hot springs, I am hoping that helps, what do you think of that.

.@@totaltherapysolutions today is day one of week 5 after TKR and I am still. using a. cane and my leg is super tight, especially above the knee.It seems that no matter what I do for rehab, it will not. loosen up. The tissue on top of and above the knee cap is hard, like harness leather and will not stretch no matter what I do. I use ice pack (Power Play compression pack) and heat, and massage with two tennis balls taped together., and heel slides, forward stretching lunges on the staircase, and a 20 minute ride on a recumbent bike, plus what the PT folks have me do there 3 times a week. Do you think ultra sound would breakup that hard tight tissue, the only thing I haven't tried yet is the mineral hot springs, which I hope to do in a few days. I am so frustrated, it seems as thoughI just can't make any progress.

Thank you for your videos. I had a total knee replacement 2 weeks ago. I have alot of tightness in the thigh. I can't bend my knee that much yet. It seems I go backwards somedays to square one.

Two steps forward and 1 step back. Sometimes it feels like three steps backward. Haha. I have a video on the rolling pin for hip and thigh pain. Massage seems to work really well for thigh pain. It is very common and does get better but massaging it helps.

Thanks for this video. I started using it tonight. My journey has been long. Had TKR, manipulation and scar tissue removal and have been trying to increase my degree of bending ever since. So and additional tips would be appreciated because the pulling with the green band and wall slide are not very beneficial. Also working on strengthening gluteus because they are some what weak. Had pt over 3 months, now on my own.

The patient you’ve described in this video sounds soo much like me ( although it’s not). I am a very fit, healthy, active little “old” lady in my 70’s… really! My TKR was the result of my literally tearing my own meniscus and ACL in several places by pushing down on my leg in the middle of the night to relieve a muscle cramp! (Did I mention that I’m also pretty strong?😆). I’ve always had a high tolerance for pain, so I didn’t realize that my knee was bone on bone. We decided to go for TKR rather than repair the new damage. I’ve been aggressive with my PT and at home, and I keep up with my ADL. But….. here I am at almost 8 wks po only at 90•. I’m scheduled for MUA on Tuesday and I’m scared that I’ll go right back to the “unconscious guarding” after I have that procedure. 😢the pain of PT is unbearable at times and, like the previous person stated, I’m afraid my almost dreaming is upsetting the other patients ( and myself!!). Thank you for being so understanding and encouraging us TKR scaredy cats. 🤗

So I shattered my knee cap and didn't have surgery. After coming out of a brace at 8 weeks my leg was stick straight, no bend.I started working on ROM and achieved 38 degrees before renting an X10 machine. After working out on the machine for almost 7 weeks I am at 80 degrees ROM, barely, it's real tight. My ortho is saying it's an MUA for me if I don't reach 90 in the next 2 weeks. Personally I have a gut feeling that my knee will eventually loosen up on it's own, just using it and working it out. Wondering what your thoughts are on this? AND super jellies of all you out there who have 100 degrees or more ROM!

The TKR was the 19th operation on the knee and I had a realy nasty case of Septic Arthritis 14 years ago, I'm finding the knee is very painful and the flex is really hard work, after 4 months I'm at 103 to 105. Everything in the knee area carries scar tissue from the tendons to the muscles and the bones were damaged too, would this contribute to the flex being slow? I was walking easily without a stick after 4 weeks could do stairs properly at the sme time, there is plenty of strength, its only the pain and flex that bothers me. I also was able to fully straighten after a few hours, my surgeon is awesome!

Many commenters and KZbinrs discuss our bodies being a system. That's great. But you keep referring to our conscious will or mind as being an active participant, for better and for worse. I'm not in the PT business, but I've happily visited PT's many times, and I noticed on my own steam that my mind plays a part. "Confidence" comes first to mind. Why does my knee act as if it's not confident in a certain move? Then, same move in a swimming pool, my knee happily bends where it wouldn't just a minute before in the poolside lounge chair. Roll with it!

I see most of the videos I have watched deal with TKR. I had multiple ligament reconstruction (PCL, PLC). Should I be watching the TKR suggestions or try to find more appropriate advice for my type of surgery. I do have physio therapist appointments once weekly and exercises I do daily. I am 5 weeks post op and have probably 10-70 degrees ROM but I have been told to avoid going over 90 and to avoid using hamstring to pull leg into flex ion for now until the donor tissue is able to strengthen.

Tony, it’s been six weeks since my TKR…I’m still having a lot of pain and my doctor says I’m over doing it. I have been very faithful to do my exercises at home. I have been doing my exercises twice a day. But I’m up all day doing something. So, my PT and doctor says I’m over doing it. I’m over stressing about it afraid I won’t get it to where it needs to be. I’m 71 years old. How should my day be going as far as what to do. I’m still going to the hospital 3 days a week for PT. How will I know if you get this comment?

I'm at 3 weeks post TKR and disheartened at not making progress beyond 90 deg max. My knee is very swollen and the pain has me in tears when I try to exercise and when I wake in the night. If I do a disciplined exercise session, I know I'll then have unbearable pain and be barely able to shuffle along for many hours after with a very stiff joint that hardly bends. What should I do? Must I push through the pain as I'm terrified of having to go through this operation again due to not doing enough.
