In the comics, Medic explains that he tried several times to grow Demo's eye back, and he succeeded all the time, if it weren't for the fcat that his eye socket had been haunted by Merasmus' Bombinomicon. So every Halloween, Demo's eye escapes and becomes a Monster (Monoculus). At some point, Medic stopped growing it back because of this, and Demo kept the eyepatch.
