They may not know the nature of the bit, but it's not a surprise that they're on camera being ridiculed. Note how that guy seems to be sitting roughly in a similar seat to the guy in the tree video, accessible in an aisle seat... the producers know which seats are viable for which bits, and they look at the audience members who are sitting there and approach the ones who look right ahead of time. The gonorrhea guy doesn't even look pissed or embarrassed, I think he's either playing it stoic or just doesn't find it funny, which isn't that bad of a thing.
