In my case the scene took place in the cemetery, but I hadn't killed Cazador and I hadn't even bowed to Bhaal.. Can you share the save file before this scene? Thank you

How do u get Quil instead of Alfira?

Alfira must be unavailable the night the cutscene triggers. This can happen if you kill her before taking the long rest, or by knocking her out non-lethally. In my case, I cast Darkness and then knocked her out within that so she wouldn't know I'd done it & wouldn't aggro on me in future, as I wanted the Potent Robe reward she gives in Act 2 for the Moonrise tiefling rescue.
