I din't know much , but what i know SEN is somthing that you can see and can't in this game. you gained SEN by doing "Stuff" or with drug and for decreased it can use black soul or kill innocent/npc.

Sen is basically sanity as for what it does well it's getting to spoiler territory, I'll explain it below if you want to know regardless So basically the more you have it the more you see the illusion around you falling apart hence gaining means you're becoming more aware that the world is not real and going lower means you're going insane and start seeing things even worse than what the illusion is trying to show you. Well this is what I remember from like a year ago so I hope it's accurate.

This is a spoiler but basically: Sen is your sanity meter but it's reversed. The more Sen you have, the more you cope with your situation so for example Great Ones look like cute anime girls, horrible, mind-scarring areas you traverse look much better, etc. Basically Sen is your level of delusion.
