Actually, just regular timing. The QTE is probably bound to the animation, so your only reflex needs to be to react to the animation/cutscene/event starting to play. And then to time the button press correctly, without the need to actually react to the prompt (you only need to know when it will appear). Alternatively, primed reaction, but even that shouldn't be able to clear that time window by itself.

I don't have the exact numbers for the varying difficulties, but on normal it was definitely quicker than any of the preceding boss QTE's (god hand e.t.c). There's still a good amount of leeway in reaction time, about a second at least if memory serves me correctly. This was the highest difficulty so the reaction window was the smallest; I guess it's up for debate how long you define that window, but from the moment the button prompt starts circling to it completing (and thus failing the QTE) was a quarter of a second when I looked back it during editing. Kashiwagi doesn't screw around!

Unpopular opinion: this was my favorite boss in the game (although I have the felling I have seen his fighting style somewhere else), because of his speed it is so satisfying to counter him. If kashiwagui delt more damage he would have been a very hard boss to beat.
